摘要:
目的 观察心电散点图鉴别诊断房颤伴宽QRS波心动过速的临床应用价值。方法 选取2021年1月-2022年1月在我院诊治的73例房颤伴宽QRS波心动过速患者为研究对象,采用单象限心电散点图分析鉴别房颤伴宽QRS波心动过速,观察房颤伴宽 QRS 波心动过速分布情况、心电散点图不同心律失常B线斜率。结果 73例房颤伴宽QRS波患者中,持续性心房颤动占24.66%,房颤伴室早室速占20.55%,房颤伴差传占30.14%,房颤伴束支传导阻滞占9.59%,房颤伴心室起搏占15.07%;房颤伴室早室速及房颤伴心室起搏B线斜率与持续性房颤、房颤伴差传、房颤伴束支传导阻滞比较,差异有统计学意义(P<0.05);房颤伴室早室速与房颤伴心室起搏两组B线斜率比较,差异有统计学意义(P<0.05);但持续性房颤、房颤伴差传、房颤伴束支传导阻滞之间B线斜率比较,差异无统计学意义(P>0.05)。结论 心电散点图鉴别诊断房颤伴宽QRS波心动过速具有重要的临床应用价值,可准确的进行鉴别诊断,可为临床治疗提供一定的参考依据。
Abstract:
Objective To observe the clinical value of ECG scatter plot in differential diagnosis of atrial fibrillation with wide QRS tachycardia.Methods A total of 73 patients with atrial fibrillation complicated with wide QRS tachycardia treated in our hospital from January 2021 to January 2022 were selected as the research objects. Single quadrant ECG scatter plot was used to analyze and identify atrial fibrillation complicated with wide QRS tachycardia. The distribution of atrial fibrillation complicated with wide QRS tachycardia and the slope of B line of different arrhythmia in ECG scatter plot were observed.Results Among 73 patients with atrial fibrillation and wide QRS wave, persistent atrial fibrillation accounted for 24.66%, atrial fibrillation with premature ventricular tachycardia accounted for 20.55%, atrial fibrillation with poor transmission accounted for 30.14%, atrial fibrillation with bundle branch block accounted for 9.59%, atrial fibrillation with ventricular pacing accounted for 15.07%. There were significant differences in B-line slope between atrial fibrillation with premature ventricular tachycardia, atrial fibrillation with ventricular pacing and persistent atrial fibrillation, atrial fibrillation with differential transmission, atrial fibrillation with bundle branch block (P<0.05); there was statistically significant difference in the slope of B-line slope between atrial fibrillation with ventricular premature ventricular tachycardia and atrial fibrillation with ventricular pacing (P<0.05); however, there was no significant difference in B-line slope among persistent atrial fibrillation, atrial fibrillation with differential transmission and atrial fibrillation with bundle branch block (P>0.05).Conclusion ECG scatter plot in the differential diagnosis of atrial fibrillation with wide QRS tachycardia has important clinical application value, which can accurately identify the diagnosis and provide certain reference for clinical treatment.
