目的 了解新疆生产建设兵团医疗卫生资源的配置及公平性情况,为进一步优化卫生资源配置,合理制定区域卫生规划提供参考。方法 选择2020年《新疆生产建设兵团统计年鉴》和2019年《新疆生产建设兵团卫生事业报告》,取2019年年末兵团各师医疗卫生机构数、实有床位数、卫生技术人员数、执业(助理)医师数、注册护士数作为医疗卫生资源衡量指标,结合各师人口数量、地理面积和地区生产总值(GDP),利用洛伦兹曲线、基尼系数和泰尔指数从人口、地理、经济3个维度评价兵团医疗卫生资源配置的公平性。结果 各医疗卫生资源按人口、地理和经济计算的基尼系数分别在0.085~0.254、0.111~0.396和0.049~0.367;各医疗卫生资源的总泰尔指数在0.0704~0.2645,按人口分布的泰尔指数最小;地区内差异对泰尔指数的贡献率大于地区间差异,其中石乌地区对总泰尔指数贡献率最大。结论 兵团医疗卫生资源配置不均衡,按人口配置的公平性优于地理、经济配置,公平性差异主要来源于地区内差异;需加快医疗资源扩容和均衡布局,优化医疗卫生资源配置,缩小地区内资源配置差异,推动区域协调发展。
Abstract:
Objective To understand the status quo and equity of medical and health resources allocation in Xinjiang Production and Construction Corps, so as to provide reference for further optimizing the allocation of health resources and formulating regional health developing planning.Methods The Xinjiang Production & Construction Group Statistical Yearbook in 2020 and the Xinjiang Production and Construction Corps Health Report in 2019 were selected. The number of medical and health institutions, the number of actual beds, the number of health technicians, the number of practicing (assistant) doctors and the number of registered nurses in each division of the Corps at the end of 2019 were taken as the measurement indexes of medical and health resources. Combined with the population, geographical area and gross regional product (GDP) of each division, Lorenz curve, Gini coefficient and Theil index were used to evaluate the fairness of medical and health resource allocation in the Corps from three dimensions of population, geography and economy.Results The Gini coefficients of medical and health resources allocation according to the population,geography and economy were 0.085-0.254, 0.111-0.396 and 0.049-0.367 respectively. The total Theil index of health resources was 0.0704-0.2645, and the Theil index according to the population was the smallest. The contribution rate to the total Theil index within the group was greater than that between the groups. In addition, the contribution rate to the total Theil index of Shiwu regions was the largest.Conclusion The distribution of medical and health resources in Xinjiang Production and Construction Corps is unbalanced. The equity of population allocation is better than that of geography and economy. The difference of equity mainly comes from regional differences. It is necessary to speed up the expansion and balanced layout of medical resources, optimize the allocation of medical and health resources, narrow the differences in resource allocation within the region, and promote regional coordinated development.
