摘要:
目的 研究医联体护理模式对社区老年糖尿病患者自我管理能力及健康状况的影响。方法 选取2020年1月-2022年1月上高县人民医院接诊的66例老年糖尿病患者为研究对象，采用随机数字表法分为对照组和观察组，各33例。对照组给予常规护理，观察组在对照组基础上给予医联体护理模式，比较两组血糖（空腹血糖、糖化血红蛋白）指标、自我管理行为评分、自我管理能力级别、生活质量水平。结果 两组干预后空腹血糖、糖化血红蛋白水平低于干预前，且观察组低于对照组（P＜0.05）；两组干预后自我管理行为评分高于干预前，且观察组高于对照组（P＜0.05）；观察组自我管理能力级别3级占比高于对照组，1级、2级占比低于对照组（P＜0.05）；两组干预后生活质量各维度评分高于干预前，且观察组高于对照组（P＜0.05）。结论 医联体护理模式对社区老年糖尿病患者自我管理能力及健康状况均具有积极的影响，可提升患者自我管理能力，促进血糖水平控制，改善患者生活质量，值得临床应用。
Abstract:
Objective To study the effect of medical alliance nursing model on self-management ability and health status of elderly diabetes mellitus patients in community.Methods A total of 66 elderly patients with diabetes mellitus who were admitted to Shanggao County People’s Hospital from January 2020 to January 2022 were selected as the research objects. They were divided into control group and observation group by random number table method, with 33 cases in each group. The control group was given routine nursing, and the observation group was given medical alliance nursing mode on the basis of the control group. The blood glucose (fasting blood glucose, glycosylated hemoglobin) index, self-management behavior score, self-management ability level and quality of life were compared between the two groups.Results The levels of fasting blood glucose and glycosylated hemoglobin in the two groups after intervention were lower than those before intervention, and those in the observation group were lower than those in the control group (P<0.05). The self-management behavior score of the two groups after intervention were higher than those before intervention, and that in the observation group was higher than that in the control group (P<0.05). The proportion of self-management ability level 3 in the observation group was higher than that in the control group, and the proportion of level 1 and level 2 was lower than that in the control group (P<0.05). The scores of each dimension of quality of life in the two groups after intervention were higher than those before intervention, and those in the observation group were higher than those in the control group (P<0.05).Conclusion The medical alliance nursing model has a positive impact on the self-management ability and health status of elderly diabetic patients in the community, which can improve the self-management ability, promote blood glucose level control, improve the quality of life, and is worthy of clinical application.
