摘要:
目的 研究快速康复外科护理对踝关节骨折手术患者踝关节功能及并发症的影响。方法 选取2019年1月-2022年1月在我院诊治的66例踝关节骨折手术患者为研究对象，采用随机数字表法分为对照组（n=33）和观察组（n=33）。对照组应用常规护理干预，观察组在对照组基础开展快速康复外科护理干预，比较两组骨折愈合时间、首次下床时间、疼痛评分、踝关节功能评分、踝关节活动度以及并发症发生率。结果 观察组骨折愈合时间、首次下床时间均短于对照组（P＜0.05）；观察组术后3 d疼痛评分与对照组比较，差异无统计学意义（P＞0.05），术后7、14 d疼痛评分均小于对照组（P＜0.05）；术后1个月，两组踝关节功能评分均高于术后14 d，且观察组高于对照组（P＜0.05）；术后1个月，两组屈曲、背伸度均高于术后14 d，负重外翻、负重内翻度均小于术后14 d，且观察组屈曲、背伸度大于对照组，负重外翻、负重内翻度均小于对照组（P＜0.05）；随访3个月，观察组并发症发生率为6.06%，低于对照组的18.18%（P＜0.05）。结论 快速康复外科护理可促进踝关节骨折愈合，缩短首次下床时间，降低并发症发生率，减轻疼痛度，促进踝关节功能恢复，改善踝关节活动度，促进患者快速康复，值得临床应用。
Abstract:
Objective To study the effect of rapid rehabilitation surgical nursing on ankle function and complications in patients with surgery ankle joint fracture.Methods From January 2019 to January 2022, 66 patients with surgery ankle joint fracture diagnosed and treated in our hospital were selected as the research objects. They were divided into control group (n=33) and observation group (n=33) by random number table method. The control group was given routine nursing intervention, and the observation group was given rapid rehabilitation surgical nursing intervention on the basis of the control group. The fracture healing time, first time to get out of bed, pain score, ankle function score, ankle mobility and complication rate were compared between the two groups.Results The fracture healing time and the first time to get out of bed in the observation group were shorter than those in the control group (P<0.05). There was no significant difference in pain score between the observation group and the control group at 3 days after operation (P＞0.05), and the pain score at 7 and 14 days after operation was lower than that in the control group (P<0.05). At 1 month after operation, the ankle function score of the two groups was higher than that at 14 days after operation, and that in the observation group was higher than that in the control group (P<0.05). At 1 month after operation, the flexion and dorsiflexion of the two groups were higher than those at 14 days after operation, and the weight-bearing valgus and weight-bearing varus were lower than those at 14 days after operation, while the flexion and dorsiflexion of the observation group were higher than those of the control group, and the weight-bearing valgus and weight-bearing varus were lower than those of the control group (P<0.05). After 3 months of follow-up, the incidence of complications in the observation group was 6.06%, which was lower than 18.18% in the control group (P<0.05).Conclusion Rapid rehabilitation surgical nursing can promote the healing of fracture of ankle joint, shorten the time of getting out of bed for the first time, reduce the incidence of complications, relieve pain, promote the recovery of ankle joint function, improve the activity of ankle joint, and promote the rapid recovery of patients, which is worthy of clinical application.
