摘要:
目的 探究口腔种植修复治疗牙列缺损伴牙周炎对牙周指数及种植体存留的影响。方法 选取2019年1月-2020年1月在我院诊治的76例牙列缺损伴牙周炎患者为研究对象，采用随机数字表法分为对照组和观察组，各38例。对照组采用常规修复治疗，观察组采用口腔种植修复治疗，比较两组临床疗效、牙周健康指标（菌斑指数、牙周袋深度、牙龈出血指数）、口腔种植修复患者不同时间段种植体存留率、炎症因子水平[白细胞（WBC）、C-反应蛋白（CRP）]、牙体表面缺损评分。结果 观察组治疗总有效率为94.74%，高于对照组的81.58%（P＜0.05）；两组治疗后菌斑指数、牙周袋深度、牙龈出血指数均小于治疗前，且观察组小于对照组（P＜0.05）；种植6个月后种植体存留率为97.36%，与种植1、2年后的94.74%、92.11%比较，差异无统计学意义（P＞0.05）；两组治疗后WBC、CRP均低于治疗前，且观察组低于对照组（P＜0.05）；两组治疗后牙体表面缺损评分均低于治疗前，且观察组低于对照组（P＜0.05）。结论 口腔种植修复治疗牙列缺损伴牙周炎具有较高的种植体存留率，改善牙周健康指标，降低炎症因子水平、牙体表面缺损评分，具有良好的临床应用效果。
Abstract:
Objective To investigate the effect of oral implant restoration on periodontal index and implant retention in the treatment of dentition defect with periodontitis.Methods A total of 76 patients with dentition defect and periodontitis who were treated in our hospital from January 2019 to January 2020 were selected as the research objects. They were divided into control group and observation group by random number table method, with 38 patients in each group. The control group was treated with conventional repair, and the observation group was treated with oral implant restoration. The clinical efficacy, periodontal health indicators (plaque index, periodontal pocket depth, gingival bleeding index), implant retention rate, inflammatory factor levels [white blood cell (WBC), C-reactive protein (CRP)], and tooth surface defect scores were compared between the two groups.Results The total effective rate of treatment in the observation group was 94.74%, which was higher than 81.58% in the control group (P<0.05). After treatment, plaque index, periodontal pocket depth and gingival bleeding index in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). The implant survival rate at 6 months after implantation was 97.36%, which was compared with 94.74% and 92.11% in 1 and 2 years after implantation, the difference was not statistically significant (P>0.05). After treatment, WBC and CRP in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). The surface defect scores of the two groups after treatment were lower than those before treatment, and the observation group was lower than the control group (P<0.05).Conclusion Oral implant restoration has a high implant retention rate in the treatment of dentition defect with periodontitis, improves periodontal health indicators, reduces the level of inflammatory factors and the score of tooth surface defect, and has a good clinical application effect.
