摘要:
目的 了解上饶市2021年死因监测漏报现况并进行分析,为评价死因监测质量提供科学依据。方法 于2021年1月-12月采用随机整群多层抽样和捕获-再捕获方法(CRM),对2021年上饶市死因监测漏报现况进行调查和分析,对象为被调查乡镇/社区的所有家庭2021年常住人口的死亡情况,通过当地公安、村委会、民政、妇保、计生、医保或新农合搜集人口死亡情况,获得2021年上饶市居民死亡数据,与死因监测系统的网报信息进行自动和人工比对,将未匹配成功者视为漏报,然后入户调查核实漏报死亡信息后再次与死因监测系统中的网报信息进行二次自动和人工比对得出最终死亡漏报名单,依此结果计算出漏报率。结果 漏报调查共收集到7645个死亡案例,最终漏报185例,漏报率为2.42%;女性漏报率高于男性漏报率(P<0.05);因病死亡漏报率高于意外死亡漏报率(P<0.05);弋阳县监测点漏报率高于余干县、鄱阳县,且余干县、鄱阳县漏报率均高于其他监测点(P<0.05);不同年龄段漏报率比较,差异有统计学意义(P<0.05),其中5岁以下儿童漏报率最高,45~65岁人群漏报率最低。结论 上饶市2021年仍然存在死因监测漏现象,且存在性别、地区、年龄差异。应切实加强该地区死因监测工作,重视各部门之间的共享机制,以提高该地区死因监测数据的完整性,真实评价上饶市居民死亡水平。
Abstract:
Objective To understand and analyze the status of underreporting of death cause monitoring in Shangrao city in 2021, and to provide scientific basis for evaluating the quality of death surveillance.Methods From January to December 2021, random cluster multi-layer sampling and capture-recapture method (CRM) were used to investigate and analyze the underreporting status of death surveillance in Shangrao city in 2021. The objects were the deaths of permanent residents in all households in the surveyed townships/communities in 2021. The death data of residents in Shangrao city in 2021 were collected through local public security, village committees, civil affairs, maternal health care, family planning, medical insurance or new rural cooperative medical system. The data were automatically and manually compared with the network report information of the death cause monitoring system, and those who failed to match were regarded as underreporting. Then, after the household survey to verify the underreported death information, the final death underreporting list was obtained by twice automatic and manual comparison with the network report information in the death surveillance system, and the underreporting rate was calculated according to the result.Results A total of 7645 death cases were collected in the underreporting survey, and 185 cases were underreported, with the underreporting rate of 2.42%. The underreporting rate of female was higher than that of male (P<0.05). The underreporting rate of death due to illness was higher than that of accidental death (P<0.05). The underreporting rate of monitoring points in Yiyang county was higher than that in Yugan county and Poyang county, and the underreporting rates in Yugan county and Poyang county were higher than those in other monitoring points (P<0.05). There was a statistically significant difference in the under-reporting rate among different age groups (P<0.05). The underreporting rate of children under 5 years old was the highest, and the under-reporting rate of 45-65 years old was the lowest.Conclusion There is still a lack of death surveillance in Shangrao city in 2021, and there are differences in gender, region and age. It is necessary to strengthen the death surveillance in this area and attach importance to the sharing mechanism among various departments, so as to improve the integrity of the monitoring data of death causes in this area and truly evaluate the death level of residents in Shangrao city.
