摘要:
目的 研究不同剂量布比卡因腰硬联合麻醉对剖宫产产妇血液流变学影响。方法 选取2021年3月-2022年3月于我院行剖宫产的60例产妇为研究对象，均采用布比卡因腰硬联合麻醉，依据布比卡因使用剂量分为A组（6 mg）、B组（7.5 mg）、C组（10 mg），每组20例。比较三组麻醉效果、麻醉临床手术指标（麻醉起效时间、苏醒时间）、平均动脉压（MAP）和心率（HR）水平、新生儿Apgar评分、产妇疼痛（VAS）评分、不良反应发生率。结果 B组、C组麻醉优良率高于A组（P＜0.05），而B组与C组麻醉优良率比较，差异无统计学意义（P＞0.05）；A组、B组麻醉起效时间长于C组，苏醒时间短于C组（P＜0.05），且B组麻醉起效时间短于A组（P＜0.05），而B组苏醒时间与A组比较，差异无统计学意义（P＞0.05）；三组麻醉后MAP、HR均低于麻醉前，且A组、B组MAP、HR均高于C组（P＜0.05），而B组与A组MAP、HR比较，差异无统计学意义（P＞0.05）；B组、A组新生儿出生后1 min Apgar评分高于C组（P＜0.05），而B组与A组新生儿出生后1 min Apgar评分比较，差异无统计学意义（P＞0.05）；C组、B组产妇术后24 h VAS评分低于A组（P＜0.05），而C组与B组产妇术后24 h VAS评分比较，差异无统计学意义（P＞0.05）；A组、B组不良反应发生率均低于C组（P＜0.05），而B组与A组不良反应发生率比较，差异无统计学意义（P＞0.05）。结论 7.5 mg布比卡因腰硬联合麻醉对剖宫产产妇血液流变学、新生儿Apgar影响较小，不良反应发生率相对较低，苏醒时间较短，还能降低产后24 h疼痛评分。
Abstract:
Objective To study the effect of different doses of bupivacaine combined spinal-epidural anesthesia on hemorheology in parturients undergoing cesarean section.Methods From March 2021 to March 2022, 60 parturients undergoing cesarean section in our hospital were selected as the research objects. All of them were treated with bupivacaine combined spinal epidural anesthesia. According to the dosage of bupivacaine, they were divided into group A (6 mg), group B (7.5 mg) and group C (10 mg), with 20 parturients in each group. The anesthesia effect, anesthesia clinical operation index (anesthesia onset time, recovery time), mean arterial pressure (MAP) and heart rate (HR) levels, neonatal Apgar score, maternal pain (VAS) score and incidence of adverse reactions were compared among the three groups.Results The excellent and good rate of anesthesia in group B and group C was higher than that in group A (P<0.05), but there was no significant difference in the excellent and good rate of anesthesia between group B and group C (P>0.05). The onset time of anesthesia in group A and group B was longer than that in group C, and the recovery time was shorter than that in group C (P<0.05), meanwhile the onset time of anesthesia in group B was shorter than that in group A (P<0.05), but there was no significant difference in the recovery time between group B and group A (P>0.05). The MAP and HR of the three groups after anesthesia were lower than those before anesthesia, and the MAP and HR of group A and group B were higher than those of group C (P<0.05), but There was no significant difference in MAP and HR between group B and group A (P>0.05). The Apgar score at 1 min after birth in group B and group A was higher than that in group C (P<0.05), but there was no significant difference in Apgar score between group B and group A at 1 min after birth (P>0.05). The VAS scores of group C and group B at 24 h after operation were lower than those of group A (P<0.05), but there was no significant difference in VAS score between group C and group B at 24 h after operation (P>0.05). The incidence of adverse reactions in group A and group B was lower than that in group C (P<0.05), but there was no significant difference in the incidence of adverse reactions between group B and group A (P>0.05).Conclusion 7.5 mg bupivacaine combined spinal-epidural anesthesia has little effect on hemorheology and neonatal Apgar in cesarean section, meanhile the incidence of adverse reactions is relatively low, the recovery time is short, and the pain score at 24 hours after delivery can be reduced.
