摘要:
目的 研究常规检验指标对孕20周前妊娠期高血压疾病（HDP）的预测价值。方法 选择2020年1月-2022年8月乐平市妇幼保健院收治的31例HDP孕妇设为HDP组，另选取同期产检的31例正常孕妇设为健康组，比较两组孕20周前的红细胞（RBC）、血红蛋白（HGB）、血小板计数（PLT）、血尿酸（UA）、肌酐（Cr）、丙氨酸氨基转移酶（ALT）、门冬氨酸氨基转移酶（AST）水平，采用多因素Logistic回归分析HDP的独立影响因素，采用ROC分析检验指标对HDP的预测价值。结果 HDP组PLT、UA、Cr、AST水平高于健康组（P＜0.05），而两组RBC、HGB、ALT比较，差异无统计学意义（P＞0.05）。多因素Logistic分析显示，PLT、UA、Cr、AST是HDP形成的独立影响因素（P＜0.05）。经ROC曲线分析显示，PLT、UA、Cr、AST联合检验对HDP具有中等预测价值。结论 PLT、UA、Cr、AST指标对孕20周前HDP风险具有确切预测作用，且联合检验预测价值更高。
Abstract:
Objective To study the predictive value of routine test indexes in hypertensive disorders of pregnancy before 20 weeks of pregnancy.Methods Thirty-one pregnant women with HDP admitted to Leping Maternal and Child Health Hospital from January 2020 to August 2022 were set as HDP group, and 31 normal pregnant women who underwent prenatal examination during the same period were set as healthy group. The levels of red blood cell (RBC), hemoglobin (HGB), platelet count (PLT), serum uric acid (UA), creatinine (Cr), alanine aminotransferase (ALT) and aspartate aminotransferase (AST) before 20 weeks of pregnancy were compared between the two groups. Multivariate logistic regression was used to analyze the independent influencing factors of HDP, and ROC analysis was used to test the predictive value of indicators for HDP.Results The levels of PLT, UA, Cr and AST in HDP group were higher than those in healthy group (P<0.05), but there was no significant difference in RBC, HGB and ALT between the two groups (P>0.05). Multivariate Logistic analysis showed that PLT, UA, Cr and AST were independent influencing factors of HDP formation (P<0.05). ROC curve analysis showed that the combined test of PLT, UA, Cr and AST had moderate predictive value for HDP.Conclusion PLT, UA, Cr and AST indicators have a definite predictive effect on the risk of HDP before 20 weeks of pregnancy, and the combined test has a higher predictive value.
