摘要:
目的 研究不同体位护理频次在老年髋关节置换术患者中的应用效果。方法 选取2020年1月-12月我院诊治的92例老年髋关节置换术患者为研究对象，采用随机数字表法分为对照组和观察组，各46例。对照组给予常规护理，观察组在对照组基础上给予不同体位护理频次干预，比较两组凝血功能指标[凝血酶原时间（PT）、凝血酶时间（TT）、纤维蛋白原（FIB）]、应激反应指标[肾上腺素（E）、去甲肾上腺素（NE）]、术野暴露满意度、睡眠质量评分、不良反应发生率、并发症发生率。结果 两组术后PT、TT、FIB、E、NE水平均高于术前，但观察组低于对照组（P＜0.05）；观察组术野暴露满意度为97.83%，高于对照组的86.96%（P＜0.05）；观察组睡眠质量评分低于对照组（P＜0.05）；观察组不良反应发生率为6.52%，低于对照组的15.22%（P＜0.05）；观察组并发症发生率为8.70%，低于对照组的17.39%（P＜0.05）。结论 不同体位护理频次在老年髋关节置换术患者中具有良好的护理效果，可预防并发症，降低不良反应发生率，减轻不良应激反应，减小对凝血功能的影响，提高术野暴露满意度，改善患者术后睡眠质量，对患者的康复具有重要的作用。
Abstract:
Objective To study the application effect of different position nursing frequency in elderly patients with hip replacement.Methods A total of 92 elderly patients with hip replacement diagnosed and treated in our hospital from January to December 2020 were selected as the research objects. They were divided into control group and observation group by random number table method, with 46 patients in each group. The control group was given routine nursing, and the observation group was given different position nursing frequency intervention on the basis of the control group. The coagulation function indexes [prothrombin time (PT), thrombin time (TT), fibrinogen (FIB)], stress response indexes [epinephrine (E), norepinephrine (NE)], surgical field exposure satisfaction, sleep quality score, incidence of adverse reactions, and incidence of complications were compared between the two groups.Results The levels of PT, TT, FIB, E and NE in the two groups after operation were higher than those before operation, but those in the observation group were lower than those in the control group (P<0.05). The satisfaction rate of surgical field exposure in the observation group was 97.83%, which was higher than 86.96% in the control group (P<0.05). The sleep quality score of the observation group was lower than that of the control group (P<0.05). The incidence of adverse reactions in the observation group was 6.52%, which was lower than 15.22% in the control group (P<0.05). The incidence of complications in the observation group was 8.70%, which was lower than 17.39% in the control group (P<0.05).Conclusion Different position nursing frequency has a good nursing effect in elderly patients undergoing hip replacement, which can prevent complications, reduce the incidence of adverse reactions, reduce the adverse stress response, reduce the impact on coagulation function, improve the satisfaction of surgical field exposure, and improve the postoperative sleep quality of patients. It plays an important role in the rehabilitation of patients.
