摘要:
目的 研究胬肉切除联合自体角膜缘干细胞移植术（CAT）对翼状胬肉患者视力水平及预后的影响。方法 选择2018年11月-2022年11月天津市蓟州区人民医院收治的60例翼状胬肉患者为研究对象，行随机数字表法分为对照组（30例）与观察组（30例）。对照组给予胬肉切除术治疗，观察组在其基础上联合CAT治疗，比较两组视力水平、角膜散光度、术后恢复情况（角膜上皮修复时间、创面愈合时间、拆线时间）、并发症及预后情况。结果 术后3天，观察组视力水平高于术前（P＜0.05），术后1~2周，两组视力水平均高于术前，且观察组高于对照组（P＜0.05）。术后1个月及3个月时，两组角膜散光度均低于术前，且观察组角膜散光度低于对照组（P＜0.05）。观察组角膜上皮修复时间、创面愈合时间、拆线时间短于对照组（P＜0.05）。观察组并发症发生率低于对照组（P＜0.05）。两组预后情况比较，差异有统计学意义（P＜0.05）。结论 胬肉切除联合CAT治疗翼状胬肉效果理想，可有效提高患者视力水平，改善其散光度，且术后恢复快、并发症风险低，对疾病预后具有良好改善作用。
Abstract:
Objective To investigate the effect of pterygium excision combined with autologous limbal stem cell transplantation (CAT) on visual acuity and prognosis in patients with pterygium.Methods A total of 60 patients with pterygium admitted to Tianjin Jizhou District People’s Hospital from November 2018 to November 2022 were selected as the research objects and divided into control group (30 patients) and observation group (30 patients) by random number table method. The control group was treated with pterygium excision, and the observation group was treated with CAT on the basis of the control group. The visual acuity, corneal astigmatism, postoperative recovery (corneal epithelial repair time, wound healing time, suture removal time), complications and prognosis were compared between the two groups.Results At 3 days after operation, the visual acuity of the observation group was higher than that before operation (P<0.05). At 1-2 weeks after operation, the visual acuity of the two groups was higher than that before operation, and that of the observation group was higher than that of the control group (P<0.05). At 1 month and 3 months after operation, the corneal astigmatism of the two groups was lower than that before operation, and the corneal astigmatism of the observation group was lower than that of the control group (P<0.05). The corneal epithelial repair time, wound healing time and suture removal time in the observation group were shorter than those in the control group (P<0.05). The incidence of complications in the observation group was lower than that in the control group (P<0.05). There was significant difference in prognosis between the two groups (P<0.05).Conclusion Pterygium resection combined with CAT is effective in the treatment of pterygium, which can effectively improve the visual acuity of patients and improve their astigmatism. Meanwhile, it has the advantages of rapid postoperative recovery and low risk of complications, and has a good improvement effect on the prognosis of the disease.
