摘要:
甲状旁腺功能减退症(HPT)最常见的原因是甲状腺、咽喉部及其他颈部肿瘤手术损伤,也是甲状腺外科领域面临的难题。目前研究认为加强围手术期管理是预防术后HPT的关键,长期补充钙剂和维生素D是治疗永久性HPT的传统方法,但可能诱发异位钙化及肾功能下降等并发症,导致部分患者丧失劳动能力,影响生活质量和长期预后。近年来,重组人甲状旁腺多肽类似物替代治疗和甲状旁腺异体移植及干细胞联合移植技术可能成为非常有前景的治疗手段。本文归纳总结甲状腺外科手术后发生HPT的原因及诱发因素,并针对近年来研究发现可能行之有效的预防方法及治疗策略进行综述,旨在为临床预防及治疗HPT提供新思路。
Abstract:
Hypoparathyroidism (HPT) is usually caused by surgical injuries of thyroid, throat and other head-neck tumors, and it is also a urgent problem in the field of thyroid surgery. Strengthening perioperative management is the key to prevent postoperative hypoparathyroidism. As a traditional treatment, long-term supplementation of calcium and vitamin D may induce complications such as ectopic calcification and decreased renal function, which may lead to loss of working ability in some patients and affect the quality of life and long-term prognosis. In recent years, recombinant human parathyroid polypeptide replacement, parathyroid allotransplantation and stem cell cotransplantation technology may become very promising therapeutic strategies. This article summarizes the causes and inducing factors of HPT after thyroid surgery, and reviews the possible effective prevention methods and treatment strategies found in recent years, aiming to provide new ideas for clinical prevention and treatment of HPT.
