摘要:
目的 分析不同性别早发冠心病（PCAD）患者心血管危险因素的分布特点。方法 选择2016年5月~2020年1月我院经冠状动脉造影明确诊断冠心病的1575例患者，按照年龄分层，对不同性别PCAD患者的传统心血管危险因素吸烟、超重/肥胖、高脂血症、高血压、糖尿病、冠心病家族史进行分析。结果 男性早发冠心病BMI水平、肥胖、吸烟、冠心病家族史比例高于女性，合并高血压及糖尿病比例低于女性，差异均有统计学意义（P＜0.05）；男女患者高脂血症比例比较，差异无统计学意义（P＞0.05）；不同年龄段男性合并高脂血症、吸烟、冠心病家族史的比例比较，差异无统计学意义（P＞0.05）；不同年龄段男性合并超重/肥胖比例随年龄增长呈下降趋势，合并糖尿病、高血压比例随年龄增长而增加，差异有统计学意义（P＜0.05）；不同年龄段女性合并高血压、高脂血症、糖尿病、冠心病家族史的比例比较，差异无统计学意义（P＞0.05）；不同年龄段女性合并超重/肥胖比例随年龄增加呈下降趋势，差异有统计学意义（P＜0.05）；当PCAD患者仅存在一个传统危险因素时，男性吸烟比例最高，女性高血压比例最高。结论 高脂血症为男、女性患者早发冠心病最常见的传统心血管危险因素；男性吸烟为常见危险因素，女性高血压为常见危险因素，超重/肥胖是年轻男女性发病患者的主要危险因素。
Abstract:
Objective To analyze the distribution characteristics of cardiovascular risk factors in patients with premature coronary heart disease (PCAD) of different genders.Methods A total of 1,575 patients with coronary heart disease diagnosed by coronary angiography in our hospital from May 2016 to January 2020 were selected.According to age stratification, the traditional cardiovascular risk factors of smoking, overweight/obesity, hyperlipidemia, hypertension, diabetes, and family history of coronary heart disease in PCAD patients of different genders were analyzed.Results The proportion of men with premature coronary heart disease BMI level, obesity, smoking, and family history of coronary heart disease was higher than that of women, and the proportion of combined hypertension and diabetes was lower than that of women, the difference was statistically significant (P<0.05);There was no significant difference in the proportion of male and female patients with hyperlipidemia (P>0.05);There was no significant difference in the proportion of males of different ages with family history of hyperlipidemia, smoking, and coronary heart disease (P>0.05);The proportion of men with overweight/obesity in different age groups showed a downward trend with age, and the proportion of men with diabetes and hypertension increased with age, the difference was statistically significant (P<0.05);There was no significant difference in the proportion of women of different ages with family history of hypertension, hyperlipidemia, diabetes, and coronary heart disease (P>0.05);The proportion of women with overweight/obesity in different age groups showed a downward trend with age,the difference was statistically significant (P＜0.05);When there was only one traditional risk factor in PCAD patients, the proportion of male smoking was the highest, and the proportion of female hypertension was the highest.Conclusion Hyperlipidemia is the most common traditional cardiovascular risk factor for early-onset coronary heart disease in men and women; smoking is a common risk factor for men, hypertension is a common risk factor for women, and overweight/obesity is the main risk for young men and women with onset factor.
