摘要:
目的 研究高通量血液透析对尿毒症患者的钙磷代谢、微炎症状态及尿素透析充分性的影响。方法 选取2020年1月-2022年1月在我院诊治的84例尿毒症患者为研究对象,采用随机数字表法分为对照组和观察组,各42例。对照组采用低通量血液透析,观察组实施高通量血液透析,比较两组血钙水平、血磷水平、炎症因子指标水平、尿素透析充分性指标、肾功能指标以及并发症发生率。结果 两组治疗后血钙水平均高于治疗前,血磷水平均低于治疗前,且观察组血钙水平高于对照组,血磷水平低于对照组(P<0.05);两组治疗后超敏-C反应蛋白(hs-CRP)、白细胞介素6(IL-6)均低于治疗前,且观察组低于对照组(P<0.05);观察组尿素清除指数(Kt/V)均高于对照组(P<0.05);两组治疗后血肌酐(SCr)、血清尿素氮(BUN)均低于治疗前,且观察组低于对照组(P<0.05);观察组并发症发生率为7.14%,低于对照组16.67%(P<0.05)。结论 高通量血液透析对尿毒症患者的钙磷代谢微炎症状具有积极的影响,可改善血钙、血磷水平,抑制微炎反应,改善肾功能,降低并发症发生率,提高尿素透析充分性,相对具有更优的应用优势。
Abstract:
Objective To study the effects of high flux hemodialysis on calcium and phosphorus metabolism, microinflammatory state and urea dialysis adequacy in uremia patients.Methods A total of 84 uremic patients diagnosed and treated in our hospital from January 2020 to January 2022 were selected as the research objects. They were divided into control group and observation group by random number table method, with 42 cases in each group. The control group was treated with low flux hemodialysis, and the observation group was treated with high flux hemodialysis. The blood calcium level, blood phosphorus level, inflammatory factor index level, urea dialysis adequacy index, renal function index and complication rate were compared between the two groups.Results After treatment, the blood calcium levels of the two groups were higher than those before treatment, and the blood phosphorus levels were lower than those before treatment, while the blood calcium level of the observation group was higher than that of the control group, and the blood phosphorus level was lower than that of the control group (P<0.05). After treatment, the levels of hypersensitive C-reactive protein (hs-CRP) and interleukin-6 (IL-6) in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). The urea clearance index (Kt/V) in the observation group was higher than that in the control group (P<0.05). After treatment, serum creatinine (SCr) and serum urea nitrogen (BUN) in the two groups were lower than those before treatment, and those in the observation group were lower than those in the control group (P<0.05). The incidence of complications in the observation group was 7.14%, which was lower than 16.67% in the control group (P<0.05).Conclusion High flux hemodialysis has a positive effect on the calcium-phosphorus metabolism microinflammation in uremic patients, which can improve blood calcium and phosphorus levels, inhibit micro-inflammatory reactions, improve renal function, reduce the incidence of complications, and improve urea dialysis adequacy. It has relatively better application advantages.
