摘要:
目的 研究托法替布治疗对类风湿关节炎患者腱鞘厚度、滑膜厚度的作用。方法 选取2021年7月-2022年7月在我院诊治的74例类风湿关节炎患者为研究对象，采用随机数字表法分为对照组和观察组，各组37例。对照组采用甲氨蝶呤+来氟米特治疗，观察组采用甲氨蝶呤+托法替布治疗。比较两组临床疗效、临床症状改善指标（关节肿胀数、关节压痛数、晨僵时间）、疼痛（VAS）评分、DAS28评分、腱鞘和滑膜厚度以及不良反应发生率。结果 观察组治疗总有效率（94.59%）高于对照组（81.08%）（P＜0.05）；观察组关节肿胀数和关节压痛数缓解数、晨僵缓解占比均高于对照组（P＜0.05）；两组治疗后VAS评分、DAS28评分均低于治疗前，且观察组评分低于对照组（P＜0.05）；两组治疗后腱鞘厚度、滑膜厚度均小于治疗前，且观察组小于对照组（P＜0.05）；观察组不良反应发生率为5.41%，低于对照组的13.51%（P＜0.05）。结论 托法替布治疗类风湿关节炎患者临床疗效良好，可减轻患者临床症状，降低疼痛评分，对腱鞘和滑膜厚度改善具有重要作用，而高频彩超可用于托法替布治疗类风湿关节炎临床有效性、安全性的评估。
Abstract:
Objective To study the effect of tofacitinib on tendon sheath thickness and synovial thickness in patients with rheumatoid arthritis.Methods A total of 74 patients with rheumatoid arthritis diagnosed and treated in our hospital from July 2021 to July 2022 were selected as the research objects. They were divided into control group and observation group by random number table method, with 37 patients in each group. The control group was treated with methotrexate+leflunomide, and the observation group was treated with methotrexate+tofacitinib. The clinical efficacy, clinical symptom improvement indicators (joint swelling number, joint tenderness number, morning stiffness time), pain (VAS) score, DAS28 score, tendon sheath and synovial thickness, and incidence of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group (94.59%) was higher than that in the control group (81.08%) (P<0.05). The number of joint swelling, the number of joint tenderness relief and the proportion of morning stiffness relief in the observation group were higher than those in the control group (P<0.05). After treatment, the VAS score and DAS28 score of the two groups were lower than those before treatment, and the scores of the observation group were lower than those of the control group (P<0.05). After treatment, the thickness of tendon sheath and synovial membrane in the two groups were smaller than those before treatment, and those in the observation group were smaller than those in the control group (P<0.05). The incidence of adverse reactions in the observation group was 5.41%, which was lower than 13.51% in the control group (P<0.05).Conclusion Tofacitinib has a good clinical effect in the treatment of patients with rheumatoid arthritis, which can reduce the clinical symptoms and pain scores of patients, and plays an important role in improving the thickness of tendon sheath and synovium. High-frequency color Doppler ultrasound can be used to evaluate the clinical efficacy and safety of tofacitinib in the treatment of rheumatoid arthritis.
