摘要:
目的 探讨慢性乙型病毒性肝炎（CHB）患者外周血Th1/Th2相关细胞因子表达水平及临床意义。方法 选取2022年1月-2023年2月大理大学第二附属医院收治的56例CHB患者为研究组，另选取同时期31名体检健康的同龄者为对照组；依据乙肝两对半检测结果将研究组分为HBeAg（+）组与HBeAg（-）组，以病毒载量104拷贝/ml为界值，将HBeAg（+）组进一步分为高病毒载量组（＞104拷贝/ml）与低病毒载量组（＜104拷贝/ml）。检测研究组与对照组外周血Th1/Th2相关细胞因子表达水平，并比较研究组中HBeAg（+）组与HBeAg（-）组Th1/Th2相关细胞因子检测结果，进一步分析HBeAg（+）组中的高病毒载量组与低病毒载量组Th1/Th2相关细胞因子水平差异。结果 研究组外周血IFN-y、IL-2、IL-4、IL-5水平均高于对照组，差异有统计学意义（P＜0.05）；研究组中HBeAg（+）组外周血IFN-γ和IL-2水平低于HBeAg（-）组，而外周血IL-4和IL-5水平高于HBeAg（-）组，差异有统计学意义（P＜0.05）；HBeAg（+）组中低病毒载量组外周血IFN-γ、IL-2、IL-4和IL-5水平均低于高病毒载量组，差异有统计学意义（P＜0.05）。结论 CHB患者存在免疫调节功能紊乱，检测CHB患者外周血Th1/Th2相关细胞因子表达水平对疾病的诊断、病情严重程度的判断及预后具有重要的参考价值。
Abstract:
bjective To investigate the expression level and clinical significance of Th1/Th2 related cytokines in peripheral blood of patients with chronic hepatitis B (CHB).Methods A total of 56 CHB patients admitted to the Second Affiliated Hospital of Dali University from January 2022 to February 2023 were selected as the study group, and 31 healthy peers at the same time were selected as the control group. The study group was divided into HBeAg(+) group and HBeAg(-) group according to the results of two pairs of semi-hepatitis B test. With the viral load of 104 copies/ml as the boundary value, the HBeAg(+) group was further divided into high viral load group (>104 copies/ml) and low viral load group (<104 copies/ml). The expression levels of Th1/Th2-related cytokines in peripheral blood of the study group and the control group were detected, and the results of Th1/Th2-related cytokines in HBeAg(+) group and HBeAg(-) group were compared. The difference of Th1/Th2-related cytokine levels between high viral load group and low viral load group in HBeAg(+) group was further analyzed.Results The levels of IFN-y, IL-2, IL-4 and IL-5 in peripheral blood of the study group were higher than those of the control group, and the differences were statistically significant (P<0.05). In the study group, the levels of IFN-γ and IL-2 in peripheral blood of HBeAg(+) group were lower than those in HBeAg(-) group, while the levels of IL-4 and IL-5 in peripheral blood were higher than those in HBeAg(-) group, the differences were statistically significant (P<0.05). The levels of IFN-γ, IL-2, IL-4 and IL-5 in peripheral blood of low viral load group in HBeAg(+) group were lower than those in high viral load group, and the differences were statistically significant (P<0.05).Conclusion CHB patients have immune regulation dysfunction. The detection of Th1/Th2 related cytokines in peripheral blood of CHB patients has important reference value for the diagnosis, severity and prognosis of the disease.
