摘要:
目的 研究糖化血红蛋白(HbA1c)联合血脂指标检测对糖尿病的诊断及预测价值。方法 以2021年1月-2023年1月宁都县中医院收治的60例糖尿病患者设为糖尿病组,另取同期体检健康者50名设为健康组,比较两组HbA1c、血脂指标[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]、FPG,分析FPG、血脂及联合检测对糖尿病的诊断价值,评估糖尿病发生的独立危险因素,以及HbA1c、血脂及其联合检测对糖尿病的预测价值。结果 糖尿病组HbA1c、TC、TG、LDL-C水平高于健康组,HDL-C水平低于健康组(P<0.05)。经ROC曲线分析,HbA1c检测对糖尿病具有确切诊断价值(AUC=0.805,P<0.05),但HbA1c联合血脂诊断糖尿病的AUC值高于单一HbA1c检测(P<0.05)。经Logistic多因素分析,HbA1c(OR=2.197,P<0.001)、TC(OR=1.865,P=0.002)、TG(OR=1.746,P=0.005)是导致糖尿病发生的独立危险因素(P<0.05)。经ROC曲线分析,HbA1c对糖尿病具有确切预测价值(AUC=0.823,P<0.05),但HbA1c联合血脂检测预测糖尿病的AUC值高于其单一检测(P<0.05)。结论 HbA1c联合血脂指标在糖尿病检测中具有较高诊断及预测价值。
Abstract:
Objective To study the diagnostic and predictive value of glycosylated hemoglobin (HbA1c) combined with blood lipid index in diabetes mellitus.Methods A total of 60 patients with diabetes mellitus admitted to Ningdu County Hospital of Traditional Chinese Medicine from January 2021 to January 2023 were selected as the diabetes group, and 50 healthy subjects were selected as the healthy group. HbA1c, blood lipid indexes [total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol(LDL-C)] and FPG were compared between the two groups. The diagnostic value of FPG, blood lipid and combined detection for diabetes was analyzed. The independent risk factors of diabetes and the predictive value of HbA1c, blood lipid and combined detection for diabetes mellitus were evaluated.Results The levels of HbA1c, TC, TG and LDL-C in the diabetic group were higher than those in the healthy group, and the level of HDL-C was lower than that in the healthy group (P<0.05). According to ROC curve analysis, HbA1c detection had a definite diagnostic value for diabetes (AUC=0.805, P<0.05), but the AUC value of HbA1c combined with blood lipid in the diagnosis of diabetes was higher than that of single HbA1 c detection (P<0.05). Logistic multivariate analysis showed that HbA1c (OR=2.197, P<0.001), TC (OR=1.865, P=0.002) and TG (OR=1.746, P=0.005) were independent risk factors for diabetes (P<0.05). According to ROC curve analysis, HbA1c had an exact predictive value for diabetes (AUC=0.823, P<0.05), but the AUC value of HbA1 c combined with blood lipid detection in predicting diabetes was higher than that of single detection (P<0.05).Conclusion HbA1c combined with blood lipid index has high diagnostic and predictive value in diabetes mellitus detection.
