摘要:
目的 研究血清γ-谷氨酰转肽酶水平与冠状动脉病变严重程度的关系。方法 选取2021年5月-2022年5月在天津市第五中心医院经冠状动脉造影确诊的63例冠心病患者为观察组,并选取同期该院体检健康者60名为对照组。比较两组γ-GGT水平、不同病变血管支数(单支、双支、3支以上)冠心病患者Gensini评分和γ-GGT水平、不同Gensini评分(<41分、41~80分、>80分)冠状动脉病变患者γ-GGT水平、不同Gensini评分患者单因素分析、多因素Logistic回归分析Gensini评分危险因素,以及γ-GGT与Gensini评分的相关性。结果 观察组γ-GGT水平高于对照组(P<0.05)。单支、双支,以及3支以上冠状动脉病变患者γ-GGT水平和Gensini评分均高于对照组,且3支以上患者高于单支、双支病变患者,双支患者高于单支病变患者(P<0.05)。Gensini评分>80分冠心病患者γ-GGT水平高于评分<41分及41~80分患者,且41~80分患者高于评分<41分患者(P<0.05)。不同Gensini评分患者性别、合并症、病程比较,差异无统计学意义(P>0.05);年龄、病变血管支数比较,差异有统计学意义(P<0.05)。多因素Logistic回归分析,γ-GGT水平、年龄、病变血管支数是Gensini评分危险因素。γ-GGT水平与Gensini评分呈正相关(P<0.05)。结论 γ-GGT参与冠心病的发生与发展,检测γ-GGT水平可一定程度评估冠状动脉病变程度,且随病变血管支数增加而升高。同时γ-GGT水平与Gensini评分呈正相关,进一步为临床判断冠心病病情提供参考依据。
Abstract:
Objective To study the relationship between serum γ-glutamyl transpeptidase level and the severity of coronary artery disease.Methods A total of 63 patients with coronary heart disease diagnosed by coronary angiography in Tianjin Fifth Central Hospital from May 2021 to May 2022 were selected as the observation group, and 60 healthy people in the same period were selected as the control group. The levels of γ-GGT, Gensini scores and γ-GGT levels in patients with coronary heart disease with different number of diseased vessels (single, double, and more than 3 vessels), γ-GGT levels in patients with coronary artery disease with different Gensini scores (<41 scores, 41-80 scores, >80 scores), univariate analysis and multivariate logistic regression analysis the risk factors of patients with different Gensini scores, and the correlation between γ-GGT and Gensini score were compared between the two groups.Results The γ-GGT level in the observation group was higher than that in the control group (P<0.05). The γ-GGT level and Gensini score in patients with single, double and more than 3 coronary artery lesions were higher than those in the control group, and those in patients with more than 3 coronary artery lesions were higher than those in patients with single and double coronary artery lesions, and those in patients with double coronary artery lesions were higher than those in patients with single coronary artery lesions (P<0.05). The γ-GGT level in patients with coronary heart disease with Gensini score >80 scores was higher than that in patients with Gensini score <41 scores and 41-80 scores, and that in patients with Gensini score 41-80 scores was higher than that in patients with Gensini score <41 scores (P<0.05). There was no significant difference in gender, complications and course of disease among patients with different Gensini scores (P>0.05). There were statistically significant differences in age and number of diseased vessels (P<0.05). Multivariate Logistic regression analysis showed that γ-GGT level, age and number of diseased vessels were risk factors for Gensini score. The level of γ-GGT was positively correlated with Gensini score (P<0.05).Conclusion γ-GGT is involved in the occurrence and development of coronary heart disease. The detection of γ-GGT level can evaluate the degree of coronary artery disease to a certain extent, and increases with the increase of the number of diseased vessels. At the same time, the level of γ-GGT was positively correlated with Gensini score, which further provided a reference for clinical judgment of coronary heart disease.
