摘要:
目的 分析股骨近端抗旋髓内钉内固定治疗股骨颈骨折合并股骨转子间骨折的临床效果。方法 选取2021年9月-2023年9月我院诊治的66例股骨颈骨折合并股骨转子间骨折患者为研究对象，采用Excel随机函数法分为常规组（33例）和研究组（33例）。常规组以动力髋螺钉治疗，研究组以股骨近端抗旋髓内钉内固定治疗，比较两组临床手术指标、术后恢复指标、髋关节功能恢复优良率、髋关节功能评分、疼痛（VAS）评分、并发症发生率。结果 与常规组比较，研究组手术切口更短，术中出血量更少，手术时间、住院时间更短（P＜0.05）；与常规组比较，研究组术后下床活动、负重以及骨折愈合时间更短（P＜0.05）；与常规组髋关节功能恢复优良率的81.82%比较，研究组髋关节功能恢复优良率更高，达93.94%（P＜0.05）；与常规组比较，研究组髋关节功能评分更高（P＜0.05），而两组VAS评分比较，差异无统计学意义（P＞0.05）；随访2个月，研究组并发症发生率与常规组比较，差异无统计学意义（P＞0.05）。结论 临床股骨颈骨折合并股骨转子间骨折采用股骨近端抗旋髓内钉内固定治疗可减小手术切口，缩短手术时间，减少术中出血量，促进髋关节功能恢复，缩短术后康复时间，且具有较高安全性。
Abstract:
Objective To analyze the clinical effect of proximal femoral anti-rotation intramedullary nail internal fixation in the treatment of femoral neck fracture combined with intertrochanteric fracture.Methods From September 2021 to September 2023, 66 patients with femoral neck fracture combined with intertrochanteric fracture in our hospital were selected as the research objects. They were divided into conventional group and study group by random number table method, with 33 patients in each group. The conventional group was treated with dynamic hip screw, and the study group was treated with proximal femoral anti-rotation intramedullary nail internal fixation. The clinical operation indexes, postoperative recovery indexes, excellent and good rate of hip joint function recovery, hip joint function score, pain (VAS) score and complication rate were compared between the two groups.Results Compared with the conventional group, the surgical incision of the study group was shorter, the intraoperative blood loss was less, the operation time and hospitalization time were shorter (P<0.05). Compared with the conventional group, the time of getting out of bed, weight-bearing and fracture healing in the study group was shorter (P<0.05). Compared with 81.82% of the excellent and good rate of hip joint function recovery in the conventional group, the excellent and good rate of hip joint function recovery in the study group was higher, reaching 93.94% (P<0.05). Compared with the conventional group, the hip function score of the study group was higher (P<0.05), while there was no significant difference in VAS score between the two groups (P>0.05). After 2 months of follow-up, there was no significant difference in the incidence of complications between the study group and the conventional group (P>0.05).Conclusion Proximal femoral anti-rotation intramedullary nail internal fixation can promote the recovery of hip joint function in patients with femoral neck fracture and intertrochanteric fracture, reduce surgical incision, shorten operation time, reduce intraoperative blood loss, and shorten postoperative recovery time, and has high safety.
