摘要:
目的 探究布地奈德、左沙丁胺醇与噻托溴铵联合治疗慢阻肺的效果及对患者呼吸功能及机体炎性反应的影响。方法 选取2022年1月-2023年12月南昌大学第二附属医院收诊的88例慢阻肺患者作为研究对象，按照随机数字表法分为对照组和研究组，每组44例。对照组应用常规治疗，研究组应用布地奈德、左沙丁胺醇与噻托溴铵联合治疗。比较两组临床疗效、肺功能[呼气流量峰值（PEF）、肺活量（VC）、用力肺活量（FVC）、第1秒用力呼气容积（FEV1）]、呼吸功能[吸气峰值压（PIP）、平均气道压（Pmean）、气道阻力（Raw）]、炎性因子水平[白细胞介素-6（IL-6）、肿瘤坏死因子-α（TNF-α）、C反应蛋白（CRP）、白细胞计数（WBC）]。结果 研究组治疗总有效率高于对照组（P＜0.05）；研究组治疗后肺功能指标水平均高于对照组（P＜0.05）；研究组PIP水平均高于对照组，而Pmean、Raw水平低于对照组（P＜0.05）；研究组机体炎性反应水平低于对照组（P＜0.05）。结论 布地奈德、左沙丁胺醇与噻托溴铵联合治疗在慢阻肺患者中的临床疗效确切，能够改善患者呼吸功能以及肺功能，减轻机体炎性反应，值得临床应用。
Abstract:
Objective To investigate the effect of budesonide,levosalbutamol and tiotropium bromide in the treatment of chronic obstructive pulmonary disease and its effect on respiratory function and inflammatory response.Methods A total of 88 patients with chronic obstructive pulmonary disease admitted to the Second Affiliated Hospital of Nanchang University from January 2022 to December 2023 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 44 patients in each group. The control group was treated with routine treatment, and the study group was treated with budesonide, levosalbutamol and tiotropium bromide. The clinical efficacy, lung function [peak expiratory flow (PEF), vital capacity (VC), forced vital capacity (FVC), forced expiratory volume in first second (FEV1)], respiratory function [peak inspiratory pressure (PIP), mean airway pressure (Pmean), airway resistance (Raw)], inflammatory factor levels [interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), C-reactive protein (CRP), white blood cell count (WBC)] were compared between the two groups.Results The total effective rate of treatment in the study group was higher than that in the control group (P<0.05). After treatment, the levels of lung function indexes in the study group were higher than those in the control group (P<0.05). The level of PIP in the study group was higher than that in the control group, while the levels of Pmean and Raw were lower than those in the control group (P<0.05). The levels of inflammatory reaction in the study group were lower than those in the control group (P<0.05).Conclusion The combination of budesonide, levosalbutamoland tiotropium bromide has a definite clinical effect in patients with chronic obstructive pulmonary disease, which can improve the respiratory function and lung function of patients and reduce the inflammatory response of the body. It is worthy of clinical application.
