摘要:
目的 研究左甲状腺素钠片联合硒酵母片对老年甲状腺功能减退症（PWH）患者甲状腺激素及血液流变学的影响。方法 纳入2020年5月-2023年5月萍乡市第二人民医院收治的60例老年PWH患者，行随机数字表法分为对照组（30例）和观察组（30例）。对照组给予左甲状腺素钠片治疗，观察组在其基础上联合硒酵母片治疗，比较两组临床疗效、甲状腺激素[游离三碘甲状腺原氨酸（FT3）、游离四碘甲状腺原氨酸（FT4）、促甲状腺激素（TSH）]、血液流变学[全血低切黏度、全血高切黏度、红细胞比容（HCT）]、不良反应。结果 观察组治疗有效率较对照组高（P＜0.05）。观察组FT3、FT4水平高于对照组，TSH、全血低切黏度、全血高切黏度、HCT水平低于对照组（P＜0.05）。两组不良反应发生率对比，差异无统计学意义（P＞0.05）。结论 左甲状腺素钠片联合硒酵母片可提高老年PWH患者的临床疗效，改善其甲状腺激素与血液流变学水平，用药安全性理想。
Abstract:
Objective To study the effect of levothyroxine sodium tablets combined with selenium yeast tablets on thyroid hormone and hemorheology in elderly patients with hypothyroidism (PWH).Methods Totally 60 elderly patients with PWH admitted to Pingxiang Second People’s Hospital from May 2020 to May 2023 were were collected, and using the random number table method, 30 patients were enrolled in the control group, while 30 patients were enrolled in the observation group. The control group was treated with levothyroxine sodium tablets, and the observation group was treated with selenium yeast tablets on the basis of the control group. The clinical efficacy, thyroid hormones [free triiodothyronine (FT3), free tetraiodothyronine (FT4), thyroid stimulating hormone (TSH)], hemorheology [whole blood low shear viscosity, whole blood high shear viscosity, hematocrit (HCT)] and adverse reactions were compared between the two groups.Results Compared with the patients in the control group, the effective rate in the observation group was higher (P<0.05). Compared with the patients in the control group after the treatment, FT3 and FT4 increased, and TSH, the whole blood low shear viscosity, whole blood high shear viscosity and HCT level were significantly lower in the observation group were lower in the observation group(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Levothyroxine sodium tablets combined with selenium yeast tablets can improve the clinical efficacy of elderly patients with PWH, improve their thyroid hormone and hemorheology levels, and have ideal drug safety.
