摘要:
目的 探讨乙肝两对半+病毒DNA定量诊断慢性乙型肝炎(慢性乙肝)的价值。方法 选取2020年1月-2023年12月泰和县人民医院收治的60例乙肝患者为研究对象,对所有患者实施乙肝两对半和DNA联合检测,分析大、小三阳占比情况,大三阳患者的乙肝病毒DNA定量检测值,比较不同肝损害乙肝病毒DNA定量检测结果。结果 小三阳(HBcAb、HBeAb、HBsAg均为阳性)占比最高(36.67%),其次是大三阳(HBcAb、HBeAg、HBsAg均为阳性)(16.67%);与其他类型患者相比,大、小三阳患者的乙肝病毒DNA定量检测数值更高,差异有统计学意义(P<0.05);轻度损害患者的DNA定量检测值低于中度损害、重度损害患者,重度损害患者的DNA定量检测值高于中度损害患者,差异有统计学意义(P=0.042)。结论 乙肝两对半联合病毒DNA定量在乙肝诊断及评估中具有重要价值,能够提高诊断准确率,为临床诊断提供重要依据,值得应用。
Abstract:
Objective To investigate the value of quantitative diagnosis of chronic hepatitis B by hepatitis B virus serological markers and virus DNA. Methods A total of 60 patients with hepatitis B admitted to Taihe County People’s Hospital from January 2020 to December 2023 were selected as the research objects. All patients were tested for hepatitis B virus serological markers and DNA. The proportion of large and small three positives was analyzed. The quantitative detection value of hepatitis B virus DNA in patients with large three positives was compared, and the quantitative detection results of hepatitis B virus DNA in different liver damage were compared. Results The small three positive (HBcAb, HBeAb, HBsAg were positive) accounted for the highest proportion (36.67%), followed by the big three positive (HBcAb, HBeAg, HBsAg were positive) (16.67%); compared with other types of patients, the quantitative detection value of hepatitis B virus DNA in patients with large and small three positive was higher, and the difference was statistically significant (P<0.05). The DNA quantitative detection value of patients with mild damage was lower than that of patients with moderate damage and severe damage, and the DNA quantitative detection value of patients with severe damage was higher than that of patients with moderate damage. The difference was statistically significant (P=0.042). Conclusion Hepatitis B virus serological markers combined with virus DNA quantification has important value in the diagnosis and evaluation of hepatitis B, which can improve the diagnostic accuracy, provide an important basis for clinical diagnosis, and is worthy of application.
