摘要:
目的 比较宏基因二代测序(mNGS)技术与传统实验室检测方法在社区获得性肺炎(CAP)病原学的诊断效能。方法 采用回顾性研究方法,收集2021年1月-2022年7月我院通过支气管镜检查肺泡灌洗液标本并行mNGS送检的173例CAP患者为研究对象,比较临床相关资料及mNGS技术与传统实验室检测方法在病原学诊断上的差异。结果 173例CAP患者中,mNGS诊断阳性150例,检出阳性率86.71%;传统病原学检查诊断阳性59例,检出阳性率34.10%;mNGS病原学检出阳性率高于传统检测方法(P<0.05)。mNGS在细菌、真菌感染、不典型病原菌和诺卡菌感染的检出阳性率高于传统方法(P<0.05);细菌感染以革兰阴性菌为主,真菌感染以烟曲霉菌主,mNGS检测出传统检测方法未检测到的2例鹦鹉热衣原体、1例军团菌及7例诺卡菌。结论 mNGS在CAP的病原学诊断效能优于传统检测方法,对CAP患者的诊疗具有重要意义。
Abstract:
Objective To compare the diagnostic efficacy of metagenomic next generation sequencing (mNGS) technology and traditional laboratory detection methods in the etiology of community-acquired pneumonia (CAP). Methods A retrospective study was conducted to collect 173 CAP patients who underwent bronchoscopy for bronchoalveolar lavage fluid specimens and mNGS examination in our hospital from January 2021 to July 2022. The clinical data and the differences between mNGS technology and traditional laboratory detection methods in etiological diagnosis were compared. Results Among the 173 CAP patients, 150 cases were diagnosed as positive by mNGS, and the positive rate was 86.71%; 59 cases were diagnosed as positive by traditional etiological examination, and the positive rate was 34.10%, the positive rate of pathogenic detection of mNGS was higher than that of traditional detection methods (P<0.05). The positive detection rate of mNGS in bacterial, fungal infection, atypical pathogenic bacteria and Nocardia infection was higher than that of traditional methods (P<0.05). Bacterial infections were mainly due to Gram-negative bacteria, while the most common fungal infection was caused by Aspergillus fumigatus.. mNGS detected 2 cases of Chlamydia psittaci, 1 case of Legionella and 7 cases of Nocardia that were not detected by traditional detection methods. Conclusion mNGS is superior to traditional detection methods in the etiological diagnosis of CAP, which is of great significance for the diagnosis and treatment of CAP patients.
