摘要:
目的 探讨乙肝病毒大蛋白（HBV-LP）联合肝功能检测在孕产妇中的应用价值。方法 选取2023年1月-6月在赣州市妇幼保健院进行体检或产检的92例妊娠适龄女性作为研究对象，分为妊娠期组（46例）和非妊娠期组（46例）。根据HBV-LP感染情况，将两组分为妊娠期HBV-LP阳性组、妊娠期HBV-LP阴性组、非妊娠期HBV-LP阳性组、非妊娠期HBV-LP阴性组。采集各组空腹静脉血，比较HBV-LP阳性率及肝功能指标[总胆红素（TBIL）、谷丙转氨酶（ALT）、谷草转氨酶（AST）、总胆汁酸（TBA）、白蛋白（ALB）]水平，并分析HBV-LP表达与肝功能指标之间的相关性。结果 妊娠期组与非妊娠期组HBV-LP阳性率比较，差异无统计学意义（P＞0.05）。妊娠期HBV-LP阳性组的TBIL、ALT、AST、TBA水平均高于妊娠期HBV-LP阴性组、非妊娠期HBV-LP阳性组、非妊娠期HBV-LP阴性组，而ALB水平低于妊娠期HBV-LP阴性组、非妊娠期HBV-LP阳性组、非妊娠期HBV-LP阴性组（P＜0.05）。HNV-LP水平与TBIL、ALT、AST、TBA呈正相关，与ALB呈负相关（P＜0.05）。结论 HBV-LP联合肝功能检测对评估孕产妇肝脏健康状况具有重要意义，可为临床诊断和管理提供参考依据。
Abstract:
Objective To explore the application value of hepatitis B virus large protein (HBV-LP) combined with liver function test in pregnant women. Methods A total of 92 pregnant women of childbearing age who underwent physical examination or prenatal examination in Ganzhou Maternal and Child Health Hospital from January to June 2023 were selected as the research objects. They were divided into pregnancy group (46 pregnant women) and non-pregnancy group (46 pregnant women). According to the infection of HBV-LP, the two groups were divided into pregnancy HBV-LP positive group, pregnancy HBV-LP negative group, non-pregnancy HBV-LP positive group and non-pregnancy HBV-LP negative group. Fasting venous blood was collected from each group to compare the positive rate of HBV-LP and the levels of liver function indexes [total bilirubin (TBIL), alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bile acid (TBA), albumin (ALB)], and the correlation between HBV-LP expression and liver function indexes was analyzed. Results There was no significant difference in the positive rate of HBV-LP between the pregnancy group and the non-pregnancy group (P>0.05). The levels of TBIL, ALT, AST and TBA in the pregnancy HBV-LP positive group were higher than those in the pregnancy HBV-LP negative group, non-pregnancy HBV-LP positive group and non-pregnancy HBV-LP negative group, while the ALB level was lower than that in the pregnancy HBV-LP negative group, non-pregnancy HBV-LP positive group and non-pregnancy HBV-LP negative group (P<0.05). The level of hepatitis B virus large protein was positively correlated with TBIL, ALT, AST and TBA, and negatively correlated with ALB (P<0.05). Conclusion HBV-LP combined with liver function test is of great significance in evaluating the liver health status of pregnant women, which can provide reference for clinical diagnosis and management.
