摘要:
目的 分析脑出血患者采用集束化护理对其康复效果、不良反应的影响。方法 以随机数字表法将2021年5月-2023年8月上高县人民医院收治的62例脑出血患者分为两组，命名为对照组和观察组，各31例。对照组给予常规护理模式，观察组实施集束化护理模式。比较两组康复效果（上肢FMA评分、下肢FMA评分、MBI评分）、凝血功能（血浆纤维蛋白原、活化部分凝血活酶时间、凝血酶原时间、凝血酶时间）、护理满意度、不良反应（肺部感染、骶尾部压疮、泌尿感染、痉挛）发生率。结果 两组护理后上肢FMA评分、下肢FMA评分、MBI评分均高于护理前，且观察组康复效果优于对照组（P＜0.05）；两组护理后血浆纤维蛋白原水平均低于护理前，活化部分凝血活酶时间、凝血酶原时间、凝血酶时间短于护理前，且观察组凝血功能指标均优于对照组（P＜0.05）；与对照组护理满意度的74.19%比较，观察组护理满意度更高，占比为96.77%（P＜0.05）；与对照组不良反应发生率的22.58%比较， 观察组不良反应发生率更低，占比为3.23%（P＜0.05）。结论 集束化护理可提升脑出血患者康复效果，改善其凝血功能，且不良反应较少，患者满意度较高。
Abstract:
Objective To analyze the effect of cluster nursing on rehabilitation and adverse reactions in patients with intracerebral hemorrhage. Methods Using the random number table method, totally 62 patients with intracerebral hemorrhage admitted to Shanggao County People′s Hospital from May 2021 to August 2023 were divided into two groups, named as control group and observation group, with 31 patients in each group. The control group was given routine nursing mode, and the observation group was given cluster nursing mode. The rehabilitation effects (upper limb FMA score, lower limb FMA score, MBI score), coagulation function (plasma fibrinogen, activated partial thromboplastin time, prothrombin time, thrombin time), nursing satisfaction, and incidence of adverse reactions (pulmonary infection, sacrococcygeal pressure sore, urinary infection, spasm) were compared between the two groups. Results The FMA score of upper limbs, FMA score of lower limbs and MBI score of the two groups after nursing were higher than those before nursing, and the rehabilitation effect of the observation group was better than that of the control group (P<0.05). After nursing, the plasma fibrinogen level of the two groups was lower than that before nursing, the activated partial thromboplastin time, prothrombin time and thrombin time were shorter than those before nursing, and the coagulation function indexes of the observation group were better than those of the control group (P<0.05). Compared with 74.19% of the nursing satisfaction of the control group, the observation group had higher the nursing satisfaction, accounting for 96.77% (P<0.05). Compared with 22.58% of the control group, the observation group had lower the incidence of adverse reactions, accounting for 3.23% (P<0.05). Conclusion Cluster nursing can improve the rehabilitation effect of patients with intracerebral hemorrhage, improve the coagulation function, with fewer adverse reactions and higher patient satisfaction.
