摘要:
目的 比较后腹腔镜与开放手术行肾蒂淋巴管结扎术治疗乳糜尿的临床效果。方法 回顾分析2017年1月~2018年6月在我院治疗的124例乳糜尿患者临床资料，根据手术方式分为观察组和对照组，各62例。对照组行开放手术肾蒂淋巴管结扎术，观察组采用后腹腔镜治疗。比较两组患者手术时间、手术出血量、胃肠功能恢复时间、住院时间、术后并发症及复发情况。结果 观察组手术时间、术中出血量、术后下床时间、胃肠功能恢复时间、住院时间均低于对照组，差异有统计学意义（P＜0.05）；观察组术后并发症发生率为1.61%、复发率为0，均低于对照组的11.29%、4.83%，差异有统计学意义（P＜0.05）。结论 后腹腔镜行肾蒂淋巴管结扎术治疗乳糜尿具有创伤小、患者恢复快、并发症少、复发率低等优点，应用效果较好。
Abstract:
Objective To compare the clinical effects of laparoscopic and open surgery for renal pedicle lymphatic ligation in the treatment of chyluria. Methods The clinical data of 124 patients with chyluria treated in our hospital from January 2017 to June 2018 were retrospectively analyzed. According to the surgical method, the observation group and the control group were divided into 62 cases. The control group underwent open surgery for renal pedicle lymphatic ligation, and the observation group was treated with retroperitoneal laparoscopic surgery. The operation time, the amount of surgical bleeding, the recovery time of gastrointestinal function, the length of hospital stay, postoperative complications and recurrence were compared between the two groups.Results The operation time, intraoperative blood loss, postoperative bedtime, gastrointestinal function recovery time and hospitalization time were lower in the observation group than in the control group,the difference was statistically significant (P<0.05). The postoperative complication rate in the observation group The score was 1.61% and the recurrence rate was 0, which was lower than 11.29% and 4.83% of the control group,the difference was statistically significant (P<0.05).Conclusion Retroperitoneal laparoscopic lymphatic vessel ligation for chyluria has the advantages of less trauma, rapid recovery, less complications and low recurrence rate.
