摘要:
目的 观察化瘀定痛液联合低分子肝素治疗老年粗隆间骨折围手术期肿痛的效果，为老年粗隆间骨折围手术期管理提供参考依据。方法 选取2017年7月~2019年3月我院收治的60例老年粗隆间骨折患者作为研究对象，随机分为观察组和对照组，各30例。对照组采用低分子肝素钠抗凝治疗，观察组在对照组的基础上口服化瘀定痛液治疗，比较两组骨折后第1、7天，术后第1、7、14天VAS评分、肿胀指数评分及D-二聚体水平。结果 骨折第1天、术后第1天两组VAS评分、肿胀指数评分比较，差异无统计学意义（P＞0.05）；骨折第7天、术后第7、14天，观察组VAS评分、肿胀指数评分均低于对照组，差异有统计学意义（P＜0.05）；两组骨折第1天D-二聚体水平比较，差异无统计学意义（P＞0.05）；骨折第7天、术后第1、7、14天，观察组D-二聚体水平均低于对照组，差异有统计学意义（P＜0.05）。结论 化瘀定痛液联合低分子肝素治疗老年粗隆间骨折能够减轻患肢围手术期肿痛，降低患者D-二聚体水平，有助于骨折术后康复。
Abstract:
Objective To observe the effect of Huayudingtong liquid combined with low-molecular-weight heparin in the treatment of perioperative swelling and pain in elderly intertrochanteric fractures, and to provide a reference basis for the perioperative management of elderly intertrochanteric fractures.Methods A total of 60 elderly patients with unilateral intertrochanteric fractures treated in our hospital from July 2017 to March 2019 were selected as the research objects and randomly divided into observation group and control group with 30 cases each. The control group was treated with low-molecular-weight heparin sodium anticoagulation, and the observation group was treated with Huayudingtong fluid on the basis of the control group. VAS scores, swelling index scores, and D-dimer levels were compared on the 1st, 7th days after the fracture, and on the 1st, 7th, and 14th days after the fracture.Results There was no significant difference in VAS scores and swelling index scores between the two groups on the first day of fracture and on the first day after surgery(P>0.05). On the 7 day, 7 and 14 days after fracture, the observation group had VAS scores and swelling index scores were lower than those in the control group,the difference was statistically significant(P<0.05). There was no significant difference in D-dimer levels between the two groups on the first day of fracture(P>0.05).On the 7th day of the fracture and on the 1st, 7th, and 14th days after the operation, the D-dimer level in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Huayudingtong liquid combined with low-molecular-weight heparin in the treatment of elderly intertrochanteric fractures can alleviate the perioperative swelling and pain of the affected limb, reduce the level of D-dimer in patients, and help to recover from fractures.
