摘要:
目的 比较医疗保健相关性肺炎(HCAP)与社区获得性肺炎(CAP)的1个月内死亡人数、机械通气人数、病原学特征。方法 计算机检索中国期刊网、万方、维普等数据库,所有文献检索时限截至2018年3月,采用RevMan 5.3软件进行Meta分析。结果 纳入21个病例对照试验,共3645例患者。Meta分析结果显示,HCAP1个月内死亡人数、机械通气人数多于CAP,HCAP感染金黄色葡萄球菌、铜绿假单胞菌菌株数、大肠埃希菌菌株数、肺炎克雷伯杆菌菌株数、不动杆菌菌株数均多于CAP,HCAP感染肺炎链球菌株数少于CAP,差异均有统计学意义(P<0.05)。结论 通过比较1个月内死亡人数、机械通气人数及病原学感染菌株数等指标,认为在我国HCAP与CAP不同,但考虑目前相关研究仍然不足,还需开展大样本、高质量的病例对照试验进一步验证。
Abstract:
Objective To compare the number of deaths,the number of mechanical ventilation,and the etiological characteristics of healthcare-associated pneumonia(HCAP)and community-acquired pneumonia(CAP)within one month.Methods The computer searched Chinese Journal Network,Wanfang,Weipu and other databases.The time limit for all literature search was as of March 2018.Meta analysis was performed using RevMan 5.3 software.Results A total of 3645 patients were included in 21 case-control trials.Meta analysis results showed that the number of deaths and mechanical ventilation in HCAP were more than CAP in one month,HCAP infection of Staphylococcus aureus,Pseudomonas aeruginosa strains,Escherichia coli strains,Klebsiella pneumoniae strains,the number of Acinetobacter strains was more than CAP,and the number of HCAP-infected Streptococcus pneumoniae strains was less than CAP,the differences were statistically significant(P<0.05).Conclusion By comparing the number of deaths within a month,the number of mechanical ventilation and the number of pathogenic infection strains,it is considered that HCAP and CAP are different in China,but considering that the current relevant research is still insufficient,a large sample and high-quality case-control trials are needed to further verification.
