摘要:
目的 比较应用PFNA和PFN内固定治疗老年股骨粗隆间骨折的临床效果。方法 选择2017年6月~2019年6月我院收治的100例老年股骨粗隆间骨折患者作为研究对象，采用随机数字表法分为对照组和研究组，各50例。对照组采用PFN内固定治疗，研究组采用PFNA内固定治疗，比较两组手术时间、术中出血量、骨折愈合时间、住院时间、Harris髋关节功能评分、治疗优良率及并发症情况。结果 研究组术中出血量、手术时间、骨折愈合时间和住院时间均短于对照组，差异有统计学意义（P＜0.05）；研究组治疗后Harris髋关节功能评分（88.64±5.37）分，高于对照组的（79.28±4.95）分，差异有统计学意义（P＜0.05）；研究组治疗优良率为94.00%，高于对照组的80.00%，差异有统计学意义（P＜0.05）；两组并发症发生率比较，差异有统计学意义（P＜0.05）。结论 PFNA内固定治疗老年股骨粗隆间骨折的临床效果突出，手术时间短，患者术中出血量少，术后恢复快且并发症少，具有较高的优良率和安全性。
Abstract:
Objective To compare the clinical effects of PFNA and PFN internal fixation in the treatment of intertrochanteric fractures in the elderly. Methods A total of 100 elderly patients with intertrochanteric fractures of the femur who were admitted to our hospital from June 2017 to June 2019 were selected as the research objects. They were divided into the control group and the study group by random number table, with 50 cases in each group. The control group was treated with PFN internal fixation, and the study group was treated with PFNA internal fixation. The operation time, intraoperative blood loss, fracture healing time, hospital stay, Harris hip function score, excellent treatment rate and complications were compared between the two groups.Results Intraoperative blood loss, operation time, fracture healing time and hospital stay in the study group were shorter than those in the control group,the difference was statistically significant (P＜0.05); the Harris hip function score (88.64±5.37) of the study group after treatment was high compared with the control group (79.28±4.95), the difference was statistically significant (P<0.05); the treatment excellent and good rate of the study group was 94.00%, which was higher than the control group’s 80.00%,the difference was statistically significant (P<0.05); Comparing the incidence of complications between the two groups, the difference was statistically significant (P<0.05).Conclusion The clinical effect of PFNA internal fixation in the treatment of elderly femoral intertrochanteric fractures was outstanding, the operation time was short, the patient’s intraoperative blood loss was small, the postoperative recovery was quick and the complications were few, and it had a high good rate and safety.
