摘要:
目的 回顾性观察无创呼吸机通气对慢性阻塞性肺疾病合并Ⅱ型呼吸衰竭BODE的影响。方法 选取2018年3月~2020年3月入住杭州市富阳区第一人民院呼吸二科的慢性阻塞性肺疾病急性加重期合并存在Ⅱ型呼吸衰竭患者101例,分为观察组51例,对照组50例,观察组在对照组常规治疗基础上加用无创呼吸机通气治疗,治疗前后观察评价两组BMI值、mMRC分级、6MWD、FEV1%及BODE指数。结果 治疗后两组mMRC分级、6MWD、FEV1%及BODE指数均较治疗前改善,且观察组改善程度优于对照组,差异有统计学意义(P<0.05);两组治疗前后BMI值比较,差异无统计学意义(P>0.05)。结论 无创呼吸机辅助通气能够改善慢性阻塞性肺疾病急性加重合并呼吸衰竭患呼吸困难程度及运动耐力,BODE指数分级下降。
Abstract:
Objective The effect of non-invasive ventilator ventilation on chronic obstructive pulmonary disease with type Ⅱ respiratory failure BODE was retrospectively observed.Methods From March 2018 to March 2020, 101 patients with chronic obstructive pulmonary disease who were admitted to the SecondDepartment of Respiration, First People’s Hospital of Fuyang District, Hangzhou, were selected for acute exacerbation of type Ⅱ respiratory failure.Divided into an observation group of 51 cases and a control group of 50 cases. The observation group was treated with non-invasive ventilator ventilation on the basis of conventional treatment in the control group.Before and after treatment, the BMI value, mMRC grade, 6MWD, FEV1% and BODE index of the two groups were observed and evaluated.Results After treatment, the mMRC classification, 6MWD, FEV1% and BODE index of the two groups were improved compared with before treatment, and the improvement degree of the observation group was better than that of the control group, the difference was statistically significant(P<0.05);There was no statistically significant difference in BMI between the two groups before and after treatment (P>0.05).Conclusion Non-invasive ventilator assisted ventilation can improve the degree of dyspnea and exercise endurance in patients with acute exacerbation of chronic obstructive pulmonary disease combined with respiratory failure, and the BODE index grading decreases.
