摘要:
目的 探讨接受早期再灌注治疗的急性脑梗死（ACI）患者治疗前后血浆D-二聚体水平与治疗效果、早期预后的关系。方法 选取2019年6月~2020年6月就诊于江门市中心医院急诊科行早期再灌注治疗的ACI患者77例，检测病例早期再灌注治疗前后的血浆D-二聚体，探讨血浆D-二聚体水平与早期再灌注治疗前后美国国立卫生研究院卒中量表（NIHSS）评分、出院时改良Rankin评分量表（mRS）评分的关系。结果 早期再灌注治疗后NIHSS评分低于到诊时，差异有统计学意义（P＜0.05）；Spearman相关分析结果显示，到诊时D-二聚体与年龄、早期再灌注治疗前NIHSS评分、早期再灌注治疗后NIHSS评分、mRS评分呈正相关（r=0.391、0.420、0.528、0.515，P＜0.05）；Spearman相关分析结果显示，早期再灌注治疗后1 d血浆D-二聚体水平与患者年龄、早期再灌注治疗后NIHSS评分、mRS评分呈正相关（r=0.336、0.418、0.408，P＜0.05），与早期再灌注治疗前后NIHSS评分差呈负相关（r=-0.284，P＜0.05）。结论 血浆D-二聚体和ACI患者的病情、早期预后相关，早期再灌注治疗后的D-二聚体与治疗效果相关，早期再灌注治疗后的D-二聚体可参考评估ACI治疗后的效果与早期预后。
Abstract:
Objective To investigate the relationship between plasma D-dimer levels before and after treatment in patients with acute cerebral infarction (ACI) receiving early reperfusion therapy, treatment effect, and early prognosis.Methods A total of 77 ACI patients who received early reperfusion therapy in the emergency department of Jiangmen Central Hospital from June 2019 to June 2020 were selected to detect the plasma D-dimer before and after the early reperfusion therapy.To explore the relationship between plasma D-dimer level and the National Institutes of Health Stroke Scale (NIHSS) score before and after early reperfusion treatment, and the modified Rankin Score Scale (mRS) score at discharge.Results The NIHSS score after early reperfusion therapy was lower than that at the time of visit, the difference was statistically significant(P＜0.05);The results of Spearman correlation analysis showed that D-dimer was positively correlated with age, NIHSS score before early reperfusion therapy, NIHSS score after early reperfusion therapy, and mRS score at the time of diagnosis (r=0.391,0.420,0.528,0.515,P＜0.05);The results of Spearman correlation analysis showed that plasma D-dimer level 1 d after early reperfusion therapy was positively correlated with the patient’s age, NIHSS score and mRS score after early reperfusion therapy(r=0.336,0.418,0.408,P＜0.05).It was negatively correlated with the NIHSS score difference before and after early reperfusion treatment (r=-0.284,P＜0.05).Conclusion Plasma D-dimer is related to the condition and early prognosis of ACI patients.The D-dimer after early reperfusion therapy is related to the treatment effect, and the D-dimer after early reperfusion therapy can refer to the evaluation of the effect and early prognosis after ACI treatment.
