摘要:
目的 观察腹腔镜阑尾切除术治疗阑尾炎的临床效果及对患者胃肠功能的影响。方法 选取2019年5月-2021年4月在我院诊治的62例阑尾炎患者为研究对象，采用随机数字表法分为对照组和观察组，各31例。对照组采用传统开腹阑尾切除术治疗，观察组采用腹腔镜阑尾切除术治疗，比较两组临床手术指标、胃肠功能恢复时间、术后不同时间点疼痛评分、炎症因子水平以及并发症发生率。结果 观察组手术时间、术中出血量、手术切口、首次下床活动时间均小于对照组（P＜0.05）；观察组肠鸣音恢复时间、首次排便时间、首次排气时间均短于对照组（P＜0.05）；观察组术后1 h、术后10 h疼痛评分均低于对照组（P＜0.05）；术后观察组C反应蛋白（CRP）、肿瘤坏死因子-α（TNF-α）水平均低于对照组（P＜0.05）；观察组并发症发生率为9.67%，低于对照组的22.58%（P＜0.05）。结论 腹腔镜阑尾切除术治疗阑尾炎效果确切，手术切口小、术中出血量少、手术时间短，可减轻对患者的创伤，从而减轻术后疼痛。同时术后患者胃肠功能恢复时间短，炎性因子水平低，并发症发生率小，具有相对更优的安全性和可行性。
Abstract:
Objective To observe the clinical effect of laparoscopic appendectomy in the treatment of appendicitis and its influence on gastrointestinal function of patients.Methods A total of 62 patients with appendicitis who were treated in our hospital from May 2019 to April 2021 were selected as the research objects. They were divided into control group and observation group by random number table method, with 31 cases in each group. The control group was treated with traditional open appendectomy, and the observation group was treated with laparoscopic appendectomy. The clinical operation indexes, recovery time of gastrointestinal function, pain scores at different time points after operation, levels of inflammatory factors and incidence of complications were compared between the two groups.Results The operation time, intraoperative blood loss, surgical incision and the first ambulation time in the observation group were lower than those in the control group (P<0.05). The recovery time of bowel sounds, the first defecation time and the first exhaust time in the observation group were shorter than those in the control group (P<0.05). The pain scores of the observation group at 1 h and 10 h after operation were lower than those of the control group (P<0.05). After operation, the levels of CRP and TNF-α in the observation group were lower than those in the control group (P<0.05). The incidence of complications in the observation group was 9.67%, which was lower than 22.58% in the control group (P<0.05).Conclusion Laparoscopic appendectomy is effective in the treatment of appendicitis. It has the advantages of small incision, less intraoperative bleeding and short operation time, which can reduce the trauma and postoperative pain. At the same time, patients with postoperative gastrointestinal function recovery time is short, the level of inflammatory factors is low, and the incidence of complications is small, have relatively better safety and feasibility.
