摘要:
目的 分析肿瘤整形技术联合保留乳房术在早期乳腺癌（BC）患者中的应用效果。方法 选取2018年1月-2021年1月我院收治的60例早期BC患者作为研究对象，采用随机数字表法分为对照组与观察组，各30例。对照组采用传统保留乳房术治疗，观察组采用肿瘤整形技术联合保留乳房术治疗，比较两组手术切除指标、术后乳房美容效果、术后并发症、预后生存情况及生活质量。结果 观察组切除肿瘤组织重量与手术切缘宽度大于对照组（P＜0.05）；观察组SOMA-LNET量表各项评分均高于对照组（P＜0.05）；观察组术后并发症发生率为16.67%，低于对照组的43.33%（P＜0.05）；两组局部复发率、再次手术率、无病生存率比较，差异无统计学意义（P＞0.05）；观察组情感维度评分低于对照组，社会/家庭维度评分高于对照组（P＜0.05），而两组FACT-B评分中生理与功能维度评分比较，差异无统计学意义（P＞0.05）。结论 肿瘤整形技术联合保留乳房术的乳腺切除范围更大，可通过术后乳房缺陷的修复，优化其外形美观度，降低术后并发症发生风险，保证患者预后生存效果的同时，提升其生活质量。
Abstract:
Objective To analyze the application effect of tumor plastic surgery combined with breast conserving surgery in patients with early breast cancer (BC).Methods A total of 60 patients with early BC admitted to our hospital from January 2018 to January 2021 were selected as subjects. They were divided into control group and observation group by random number table method, with 30 cases in each group. The control group was treated with breast conserving surgery, and the observation group was treated with tumor plastic surgery combined with breast conserving surgery. The surgical resection indexes, postoperative breast cosmetic effect, postoperative complications, prognosis and quality of life were compared between the two groups.Results The tumor tissue weight and surgical margin width in the observation group were larger than those in the control group (P<0.05). The scores of SOMA-LNET scale in the observation group were higher than those in the control group (P<0.05). The incidence of postoperative complications in the observation group was 16.67%, which was lower than 43.33% in the control group (P<0.05). There was no significant difference in local recurrence rate, reoperation rate and disease-free survival rate between the two groups (P>0.05). The emotional dimension score of the observation group was lower than that of the control group, and the social/family dimension score was higher than that of the control group (P<0.05); while there was no significant difference in the scores of physiological and functional dimensions in the FACT-B scores between the two groups (P>0.05).Conclusion The range of breast resection with tumor plastic surgery combined with breast conserving surgery is larger. Through the repair of postoperative breast defects, the appearance aesthetics can be optimized, the risk of postoperative complications can be reduced, the prognosis and survival effect of patients can be ensured, and the quality of life can be improved.
