摘要:
目的 探讨玻璃酸钠关节腔注射联合体外冲击波（ESWT）治疗肩周炎的效果。方法 收集内蒙古医科大学第二附属医院2019年1月-2020 年1月收治的 80例肩周炎患者，将其随机分为A、B两组，每组40例。A组行关节腔注射玻璃酸钠治疗，B组行玻璃酸钠联合ESWT治疗。比较两组视觉模拟评分（VAS）、Fugl-Meyer 运动功能评分量表（FMA）、肩关节综合评价量表评分、肩关节功能恢复情况及不良反应。结果 治疗1、3、6个月两组VAS 评分、FMA评分和肩关节综合评分均较治疗前改善，差异有统计学意义（P＜0.05）；治疗1个月两组VAS 评分、FMA评分和肩关节综合评分比较，差异无统计学意义（P>0.05），治疗后3、6个月，B组VAS 评分、FMA评分和肩关节综合评分改善优于A组，差异有统计学意义（P＜0.05）；两组肩关节恢复有效率比较，差异无统计学意义（P>0.05）；B组肩关节恢复优良率高于A组，差异有统计学意义（P＜0.05）；两组均无不良反应发生。结论 玻璃酸钠关节腔注射联合ESWT治疗肩周炎疗效明显，安全可靠，值得临床应用。
Abstract:
Objective To explore the effect intra-articular injection of sodium hyaluronate combined with extracorporeal shock wave (ESWT) in the treatment of scapulohumeral periarthritis.Methods A total of 80 patients diagnosed and treated in the Second Affiliated Hospital of Inner Mongolia Medical Universityfrom January 2019 to January 2020 were randomly divided into group A and groupB, with 40 cases in each group. Group A received intra-articular injection of sodium hyaluronate, and group B received sodium hyaluronate combined with ESWT treatment. The Visual Analogue Scale (VAS), Fugl-Meyer Motor Function Scale (FMA), Shoulder Joint Comprehensive Evaluation Scale score, shoulder joint function recovery and adverse reactions were compared between the two groups.Results The VAS score, FMA score and shoulder joint comprehensive score of the two groups were improved after 1, 3 and 6 months of treatment, and the difference was statistically significant (P<0.05).There was no significant difference in VAS score, FMA score and shoulder joint comprehensive score between the two groups at 1 month after treatment (P>0.05). At 3 and 6 months after treatment, the improvement of VAS score, FMA score and shoulder joint comprehensive score in group B was better than that in group A, and the difference was statistically significant (P<0.05). There was no significant difference in the effective rate of shoulder joint recovery between the two groups (P>0.05). The excellent and good rate of shoulder joint recovery in group B was higher than that in group A, and the difference was statistically significant(P<0.05). No adverse reactions occurred in both groups.Conclusion Sodium hyaluronate intra-articular injection combined with ESWT in the treatment of periarthritis of shoulder is effective, safe and reliable, and is worthy of clinical application.
