摘要:
目的 探讨脓毒症并发急性肾损伤(AKI)患者血浆铁与中性粒细胞明胶酶相关脂质运载蛋白(NGAL)的相关性。方法 选取2019年1月-2020年6月上海市普陀区利群医院急诊医学科诊治的120例脓毒症患者,首先根据入院时是否并发AKI分为入院非AKI组65例和入院AKI组55例,另将入院非AKI组根据患者入院48 h后是否并发AKI分为48 h AKI组和48 h非AKI组,比较各组血浆铁和NGAL水平,并采用Spearman相关性分析入院非AKI组及48 h AKI组入院时、入院后48 h血浆铁与NGAL水平的关系。结果 入院AKI组入院时NGAL、血浆铁及入院后48 h NGAL高于入院非AKI组,差异有统计学意义(P<0.05);48 h AKI组与48 h非AKI组入院时、入院后48 h血浆铁、NGAL水平比较,差异有统计学意义(P<0.05);Spearman相关性分析显示,入院AKI组在入院时及入院后48 h的血浆铁与血浆NGAL呈正相关(r=0.535、0.498,P<0.05);48 h AKI组入院时、入院后48 h血浆铁与血浆NGAL呈正相关(r=0.593、0.641,P<0.05)。结论 脓毒症并发AKI患者血浆铁与NGAL可能存在一定相关性,血浆铁水平升高可能是脓毒症并发AKI的一个重要预警。
Abstract:
Objective To investigate the correlation between plasma iron and neutrophil gelatinase-associated lipocalin (NGAL) in patients with sepsis complicated with acute kidney injury (AKI).Methods A total of 120 patients with sepsis diagnosed and treated in the Department of Emergency Medicine, the Liqun Hospital of Shanghai City Putuo District from January 2019 to June 2020 were selected and divided into the admission non-AKI group (n=65) and the admission AKI group (n=55) according to whether AKI occurred at admission. The admission non-AKI group was divided into the 48 h AKI group and the 48 h non-AKI group according to whether AKI occurred at 48 h after admission. The plasma iron and NGAL levels in each group were compared. Spearman correlation was used to analyze the relationship between plasma iron and NGAL levels at admission and 48 h after admission in non-AKI group and 48 h AKI group.Results The plasma iron and NGAL levels at admission and NGAL at 48 h after admission in the admission AKI group were higher than those in the non-AKI group, and the difference was statistically significant (P<0.05). There was significant difference in plasma iron and NGAL levels at admission and 48 h after admission between 48 h AKI group and 48 h non-AKI group (P<0.05). Spearman correlation analysis showed that plasma iron was positively correlated with plasma NGAL at admission and 48 h after admission in the admission AKI group (r=0.535, 0.498, P<0.05); plasma iron was positively correlated with plasma NGAL at admission and 48 h after admission in the 48 h AKI group (r=0.593, 0.641, P<0.05).Conclusion There may be a certain correlation between plasma iron and NGAL in patients with sepsis complicated with AKI, and the increase of plasma iron level may be an important early warning of sepsis complicated with AKI.
