摘要:
目的 研究小剂量右美托咪定联合经鼻高流量氧疗(HFNC)治疗急性左心衰的临床疗效。方法 选取2019年9月-2020年10月我院接诊的130例急性心力衰竭患者为研究对象,采用随机数字表法分为对照组和观察组,各65例。对照组采用经鼻高流量氧疗治疗,观察组在对照组基础上给予小剂量右美托咪定。比较两组临床治疗呼吸和心功能指标、血气指标、治疗疗效、HFNC时间、气管插管和谵妄以及病死率。结果 两组心率(HR)、呼吸频率(RR)均低于治疗前,左室射血分数(LVEF)大于治疗前,且观察组HR、RR均低于对照组,LVEF大于对照组(P<0.05);治疗后,血氧饱和度(SpO2)、动脉血氧分压(PaO2)均高于治疗前,二氧化碳分压(PCO2)均小于治疗前,且观察组SpO2、PaO2大于对照组,PCO2小于对照组(P<0.05);观察组治疗总有效率为95.38%,高于对照组的81.53%(P<0.05);观察组HFNC时间短于对照组,气管插管、谵妄发生率均小于对照组(P<0.05);两组病死率比较,差异无统计学意义(P>0.05)。结论 小剂量右美托咪定联合经鼻高流量氧疗治疗急性心力衰竭效果确切,可提高治疗疗效,显著改善呼吸和心功能指标,提高血氧饱和度,改善呼吸困难状况,降低气管插管和谵妄率,缩短HFNC时间,且不会增加病死率,具有相对良好的有效性和安全性。
Abstract:
Objective To study the clinical efficacy of low-dose dexmedetomidine combined with transnasal high flow oxygen therapy (HFNC) in the treatment of acute left heart failure.Methods A total of 130 patients with acute heart failure admitted to our hospital from September 2019 to October 2020 were selected as the research objects. They were divided into control group and observation group by random number table method, with 65 cases in each group. The control group was treated with ransnasal high flow oxygen therapy, and the observation group was given low-dose dexmedetomidine on the basis of the control group. The respiratory and cardiac function indexes, blood gas indexes, therapeutic efficacy, HFNC time, tracheal intubation, delirium and mortality in the two groups were compared.Results The heart rate (HR) and respiratory rate (RR) of the two groups were lower than those before treatment, and the left ventricular ejection fraction (LVEF) was higher than that before treatment, while the HR and RR of the observation group were lower than those of the control group, and the LVEF was higher than that of the control group (P<0.05). After treatment, the blood oxygen saturation (SpO2) and arterial oxygen partial pressure (PaO2) of the two groups were higher than those before treatment, and the carbon dioxide partial pressure (PCO2) was lower than that before treatment, while the SpO2 and PaO2 of the observation group were higher than those of the control group, and the PCO2 was lower than that of the control group (P<0.05). The total effective rate of the observation group was 95.38%, which was higher than 81.53% of the control group (P<0.05). The HFNC time of the observation group was shorter than that of the control group, and the incidence of tracheal intubation and delirium was lower than that of the control group (P<0.05); there was no significant difference in mortality between the two groups (P>0.05).Conclusion Low-dose dexmedetomidine combined with transnasal high flow oxygen therapy is effective in the treatment of acute heart failure, which can improve the therapeutic effect, significantly improve respiratory and cardiac function indexes, increase blood oxygen saturation, improve dyspnea, reduce tracheal intubation and delirium rate, shorten HFNC time, and do not increase mortality, with relatively good efficacy and safety.
