摘要:
目的 探讨70岁以上住院慢性病老年患者的药学服务需求，为药学服务模式优化提供参考。方法 选取2018年8月~2019年8月我院240例住院老年（年龄＞70岁）慢性病患者作为研究对象，调查患者疾病类型、对药师和药学服务的接受状况及药学服务需求。结果 共240例70岁以上住院的慢性病患者，人均患病种2~7个，平均患病种（3.00±0.85）个，其中高血压病患病人数最多；240例患者中有35.00%的患者接受过药师的药学服务，接受药学服务的内容主要以用药咨询、药物推荐为主（27.50%），其次为药品配伍禁忌、药品不良反应及相关处理措施、药品存管方法。用药的药物服务需求主要以特殊药物及相关疾病的教育为主（80.83%），其次为常见药不良反应的应对、漏服药品的应对、变质药物的辨识、用药后的疗效评估、错服药物的处理、优化用药方案。结论 70岁以上住院慢性病患者存在多种慢性病并存现象，药学服务接受率较低，应加强药物及相关疾病的普及教育，提高临床药师综合素质，不断完善药物服务模式，以提升患者用药遵医性，进而优化疾病治疗效果。
Abstract:
Objective To explore the pharmaceutical service needs of elderly patients with chronic diseases inpatients over 70 years of age, and to provide a reference for the optimization of pharmaceutical service models. Methods A total of 240 elderly patients (aged> 70 years) with chronic diseases in our hospital from August 2018 to August 2019 were selected as the research objects, and the types of diseases, the acceptance of pharmacists and pharmaceutical services, and the demand for pharmaceutical services were investigated.Results A total of 240 patients with chronic diseases over 70 years of age were hospitalized with an average of 2 to 7 disease types per person (3.00±0.85), of which the number of patients with hypertension was the largest; 35.00% of 240 patients The content of pharmacist services for pharmacists is mainly drug consultation and drug recommendation (27.50%), followed by drug compatibility taboos, adverse drug reactions and related treatment measures, and drug storage methods. The demand for medication service for medications is mainly education on special medications and related diseases (80.83%), followed by coping with common adverse reactions to medications, coping with missed medications, identification of spoiled medications, evaluation of curative effects after medications, and wrong medication treatment and optimization of medication plan.Conclusion There are many chronic diseases coexisting in patients with chronic diseases over 70 years of age, and the acceptance rate of pharmacy services is low. It is necessary to strengthen the universal education of drugs and related diseases, improve the comprehensive quality of clinical pharmacists, and continuously improve the drug service model to improve the compliance of patients,thereby optimizing the effect of disease treatment.
