摘要:
目的 分析国内外疾病诊断相关分组(DRG)研究现状及热点,为DRG在国内的应用与发展提供建议。方法 以Web of Science核心合集数据库、PubMed、中国知网、万方数据知识服务平台为数据来源,检索建库至2019年6月11日的DRG相关文献,借助CiteSpace、Bibexcel文献分析软件对DRG相关文献的年代、国家、期刊、关键词进行分析。结果 根据1978~2018年的国内外论文数量显示,国内外文献数量整体呈线性上升趋势,其中1983年国外文献数量迅速上升,1985年国外的文献量达到顶峰,随后文献数量呈现每隔几年一个高峰的特征;1985~2004年国内文献数量非常少,2004年后国内文献量呈直线上升趋势,2018年国内文献数量达到顶峰。共纳入12059篇英文文献,1362篇中文文献,文献数量整体呈上升趋势。该领域文献发表最多的国家是美国。12059篇英文文献共发表在501种期刊上,1362篇中文文献共发表在315种期刊上。英文文献的聚类标签为死亡率、医院信息系统、流行病学、医院、可靠性、成本效益、糖尿病;中文文献的聚类标签为北京大学第三医院、绩效分配、住院费用、伴随疾病、医院、医保基金、病案首页、三医联动、服务效率、腺病毒载体、支付制度。国外1978~2019年,该领域突现词共有6个,近3年的突现词为readmission(再入院);我国1985~2019年,突现词共有15个,近3年的突现词为支付方式改革。结论 国内外DRG的研究热点大体一致,主要包括DRG分组影响因素,数据的收集处理,DRG在医疗费用管理及医疗服务绩效评价中的应用。国内在数据研究方面更强调数据质量,从侧面反映出目前我国医疗数据质量不高的问题。建议规范病案首页,提高病案数据质量,同时辨证使用DRG,加强医疗质量的监督。
Abstract:
Objective To analyze the current status and hotspots of DRG research at home and abroad, and to provide suggestions for the application and development of DRG in China. Methods Using the core collection database of Web of Science, PubMed, China HowNet, and Wanfang Data Knowledge Service Platform as data sources, the DRG-related literatures that were built up to June 11, 2019 were retrieved, and with the help of CiteSpace and Bibexcel document analysis software, the age, country, periodical and keywords of DRG-related documents were analyzed. Results According to the number of domestic and foreign papers from 1978 to 2018, the number of domestic and foreign documents has shown a linear upward trend. Among them, the number of foreign documents increased rapidly in 1983, and the number of foreign documents reached a peak in 1985. Subsequently, the number of documents showed a peak every few years The number of domestic documents was very small from 1985 to 2004. After 2004, the number of domestic documents increased linearly, and the number of domestic documents reached a peak in 2018. A total of 12,059 English literatures and 1,362 Chinese literatures were included, and the total number of literatures was on the rise. The country with the most published literature in this field is the United States. 12059 English articles were published in 501 journals, and 1362 Chinese documents were published in 315 journals. The clustering tags for the English literature are mortality, hospital information systems, epidemiology, hospitals, reliability, cost-effectiveness, diabetes; the clustering tags for the Chinese literature are Peking University Third Hospital, performance allocation, hospitalization costs, concomitant diseases, Hospitals, medical insurance funds, medical records front page, linkage of three doctors, service efficiency, adenovirus vector, payment system. From 1978 to 2019 abroad, there were 6 emergent words in this field, and the emergence words in the past 3 years were readmission (re-admission). In 1985-2019, there were 15 emergent words in China, and the emergence words in the past 3 years were payment method reform.Conclusion The research hotspots of DRG at home and abroad are generally the same, mainly including the influencing factors of DRG grouping, data collection and processing, and the application of DRG in medical cost management and medical service performance evaluation. Data quality is emphasized in data research, which reflects the low quality of medical data in China. It is suggested to standardize the front page of medical record, improve the quality of medical record data, and use DRG in syndrome differentiation to strengthen the supervision of medical quality.
