摘要:
目的 探讨椎旁肌间隙入路椎弓根螺钉内固定治疗胸腰椎脊柱骨折的疗效。方法 选取2017年1月~2019年5月我院就诊的胸腰椎脊柱骨折患者87例,采用随机数字表法分为对照组(43例)和观察组(44例)。对照组采用剥离椎旁肌后入路伤椎置钉短节段固定术,观察组采用椎旁肌间隙入路椎弓根螺钉内固定术,比较两组手术时间、术中出血量、下床活动时间、疼痛评分(VAS)及腰椎功能评分(ODI)。结果 观察组手术时间(59.42±5.98)min、术中出血量(7.21±2.43)ml、下床活动时间(2.93±1.05)d均低于对照组的(72.42±6.12)min、(15.21±3.65)ml、(5.68±1.47)d,差异有统计学意义(P<0.05);术后两组VAS评分、ODI均低于术前,且观察组低于对照组[(0.83±0.36)分 vs (7.48±2.26)分]、[(1.53±0.48)分 vs (11.25±3.62)分],差异有统计学意义(P<0.05)。结论 胸腰椎脊柱骨折患者采用椎旁肌间隙入路椎弓根螺钉内固定手术,手术时间短,术中出血量少,术后疼痛轻,可有效改善腰椎功能,利于患者康复。
Abstract:
Objective To investigate the therapeutic effect of pedicle screw fixation on the paraspinal muscle space for thoracolumbar spine fractures.Methods From January 2017 to May 2019, 87 patients with thoracolumbar spine fractures treated in our hospital were selected and divided into a control group(43 cases)and an observation group(44 cases)by random number table.The control group was treated with short-segment fixation of the vertebral screw after the dissection of the paravertebral muscles,and the observation group was treated with pedicle screw fixation of the paraspinal muscles.The operation time,intraoperative blood loss,and out of bed were compared between the two groups. Activity time,pain score(VAS)and lumbar function score(ODI).Results The operation time of the observation group(59.42±5.98)min,intraoperative blood loss(7.21±2.43)ml,time to get out of bed(2.93±1.05)d were all lower than that of the control group(72.42±6.12)min,(15.21±3.65)ml,(5.68±1.47)d,the difference was statistically significant(P<0.05),postoperative VAS score and ODI of both groups were lower than before operation,and observation group was lower than control group[(0.83±0.36)points vs (7.48±2.26)points],[(1.53±0.48)points vs (11.25±3.62)points],the difference was statistically significant(P<0.05).Conclusion Patients with thoracolumbar vertebral spine fractures are treated with pedicle screw fixation through the paraspinal muscle approach.The operation time is short,the amount of bleeding during the operation is small,and the postoperative pain is light.It can effectively improve the function of the lumbar spine and benefit the patient’s rehabilitation.
