摘要:
目的 利用EQ-5D对住院患者进行生命质量评价，探讨患者在疾病状态下的生命质量及相关影响因素。方法 运用EQ-5D量表及自编问卷，于2018年7~8月采用单纯随机抽样、分层和整群随机抽样抽取县级医院住院患者2394例分析其住院期间VAS评分和健康效用值情况。结果 VAS平均得分为76.16，健康效用值最高为1，最低为-0.013，平均得分为0.731。单因素分析显示，不同年龄、性别、民族、婚姻、文化程度、职业、城乡、医保类型、月平均收入、疾病类型、住院次数的患者VAS评分不同，差异有统计学意义（P＜0.05）；不同年龄、婚姻、文化程度、医保类型、月平均收入、疾病类型、住院次数的患者健康效用值不同，差异有统计学意义（P＜0.05）。多因素分析显示，年龄、文化程度影响患者的VAS评分和效用值（P＜0.01），疾病类型影响患者效用值（P＜0.01）。结论 人群在患病状态下VAS评分、健康效用值较一般状态低，且不同系统疾病对患者造成不同程度的影响，尤其是运动系统疾病和神经系统疾病患者生命质量较差。
Abstract:
Objective To evaluate the quality of life of inpatients with EQ-5D, and to explore the quality of life and related influencing factors of patients in disease states.Methods Using EQ-5D scale and self-made questionnaire, from July to August 2018, using simple random sampling, stratified and cluster random sampling, 2394 inpatients in county hospitals were selected to analyze the VAS score and health utility value during their hospitalization.Results The average VAS score was 76.16, the highest health utility value was 1, the lowest was -0.013, and the average score was 0.731. Single factor analysis showed that the VAS scores of patients with different age, gender, ethnicity, marriage, educational level, occupation, urban and rural, medical insurance type, monthly average income, disease type, and number of hospitalizations were different,the difference was statistically significant (P＜0.05); The health utility values of patients with different age, marriage, education level, medical insurance type, monthly average income, disease type, and number of hospitalizations were different,the difference was statistically significant (P＜0.05). Multivariate analysis showed that age and education level affected the patient’s VAS score and utility value (P＜0.01), and the type of disease affected the patient’s utility value (P＜0.01).Conclusion The VAS score and health utility value of the population in the diseased state are lower than the general state, and different system diseases have different effects on patients, especially the quality of life of patients with sports and neurological diseases is poor.
