摘要:
目的 观察舒适护理和呼吸功能锻炼干预在改善慢阻肺急性期患者肺功能和生活质量中的效果。方法 选取2018年10月~2019年10月我院诊治的94例慢阻肺急性期患者为研究对象，采用随机数字表法分为对照组和观察组，各47例。对照组采用常规护理，观察组在对照组基础上采用舒适护理和呼吸功能锻炼干预，比较两组肺功能指标[最大肺活量（VCmax）、1秒钟用力呼气量（FEV1）、1秒量比用力肺活量（FEV1/FVC）、呼气峰值流速（PEF）]、生活质量（SF-36）评分、6min步行距离（6MWD）、护理满意度及复发情况。结果 观察组VCmax、FEV1、FEV1/FVC、PEF均高于对照组，差异有统计学意义（P＜0.05）。观察组SF-36各维度评分均高于对照组，差异有统计学意义（P＜0.05）。观察组6MWD大于对照组，差异有统计学意义（P＜0.05）。观察组护理满意度为97.87%，高于对照组的85.10%，差异有统计学意义（P＜0.05）；随访3个月，观察组复发率为6.37%，低于对照组的14.89%，差异有统计学意义（P＜0.05）。结论 舒适护理和呼吸功能锻炼干预可改善慢阻肺急性期患者肺功能，提高患者生活质量，增强患者机体运动能力，降低复发率，且患者对该护理模式满意度较高。
Abstract:
Objective To observe the effects of comfort nursing and respiratory function exercise intervention in improving lung function and quality of life of patients with chronic obstructive pulmonary disease in the acute phase.Methods 94 patients with acute COPD diagnosed and treated in our hospital from October 2018 to October 2019 were selected as the research objects. They were divided into control group and observation group by random number table method, with 47 cases in each group.The control group used conventional nursing, and the observation group used comfort nursing and respiratory function exercise intervention on the basis of the control group.Compare the two groups of lung function indexes [maximum vital capacity (VCmax), forced expiratory volume in 1 second (FEV1), specific forced vital capacity in 1 second (FEV1/FVC), peak expiratory flow rate (PEF)], quality of life (SF- 36) Score, 6min walking distance (6MWD), nursing satisfaction and recurrence.Results The VCmax, FEV1, FEV1/FVC and PEF of the observation group were higher than those of the control group,the difference was statistically significant (P<0.05).The scores of all dimensions of SF-36 in the observation group were higher than those in the control group,the difference was statistically significant (P<0.05).The 6MWD of the observation group was greater than that of the control group,the difference was statistically significant(P<0.05).The nursing satisfaction of the observation group was 97.87%, which was higher than 85.10% of the control group,the difference was statistically significant (P<0.05);After 3 months of follow-up, the recurrence rate of the observation group was 6.37%, which was lower than the 14.89% of the control group,the difference was statistically significant (P<0.05).Conclusion Comfortable nursing and respiratory function exercise intervention can improve the lung function of patients with chronic obstructive pulmonary disease in the acute phase, improve the quality of life of the patients, enhance the exercise capacity of the patients, and reduce the recurrence rate, and the patients are more satisfied with the nursing mode.
