摘要:
目的 采用数据挖掘方法从医院总体层面分析临床用药存在的问题，为临床药师进行合理用药点评提供参考依据。方法提取某院2018年住院患者的药品使用情况，应用R软件建立关联分析模型，根据支持度和可信度得到关联规则，对有意义的关联规则进行分析，总结患者用药关联关系情况。结果 共提取10,884例住院患者，其中男性5365例，女性5519例，涉及183,775条住院药品使用记录，包括197类，739种（不含溶媒）药品；药物使用率前3位的依次是维生素C、地塞米松注射液和盐酸利多卡因注射液；住院处方前20位的药物中，抗菌药物有2种，辅助药物3种；手术和操作患者中羟乙基淀粉130/0.4氯化钠注射液、盐酸昂丹司琼注射液和乳酸钠林格注射液联合使用最多，其次是联用地塞米松注射液和注射用矛头蝮蛇血凝酶；抗菌药物使用中注射用头孢孟多酯钠和奥硝唑氯化钠注射液使用存在明显关联，辅助用药中注射用奥美拉唑钠和复方氨基酸注射液联用最为常见。结论 该院住院患者用药存在一定规律性，应用关联规则方法可对用药规律进行挖掘，为确定合理用药的点评重点提供依据。
Abstract:
Objective To use data mining method to analyze the problems of clinical medication from the overall level of the hospital, and to provide a reference for clinical pharmacists to comment on rational medication.Methods Extracting the drug usage of inpatients in a hospital in 2018, and use R software to establish an association analysis model.According to the degree of support and credibility, the association rules are obtained, the meaningful association rules were analyzed, and the patient medication association relationship was summarized.Results A total of 10,884 hospitalized patients were extracted, including 5365 males and 5519 females, involving 183,775 hospitalized drug use records, including 197 categories and 739 (without solvents) drugs;The top 3 drug usage rates were vitamin C, dexamethasone injection and lidocaine hydrochloride injection;Among the top 20 drugs in hospital prescriptions, there were 2 kinds of antibacterial drugs and 3 kinds of auxiliary drugs; hydroxyethyl starch 130/0.4 sodium chloride injection, ondansetron hydrochloride injection and sodium lactate ringer injection in surgery and operation patients liquid combination was the most used,the second was the combined use of dexamethasone injection and Agkistrodon hemagglutinin for injection;In the use of antibacterial drugs, there was a clear correlation between the use of cefamendol sodium for injection and ornidazole sodium chloride injection, and the combination of omeprazole sodium for injection and compound amino acid injection was the most common auxiliary drug.Conclusion There is a certain regularity in the medication use of inpatients in this hospital, and the application of association rules method can be used to mine the law of medication and provide a basis for determining the key points of rational use of drugs.
