摘要:
目的 研究腹腔镜下完全腹膜外疝修补术治疗成人腹股沟疝的临床疗效。方法 选取2019年4月~2020年4月我院治疗的74例成人腹股沟疝患者为研究对象，采用随机数字表法分为对照组和观察组，各37例。对照组采用开放填充式无张力疝修补术，观察组采用腹腔镜下完全腹膜外疝修补术治疗，比较两组手术指标、VAS评分、并发症发生率以及复发率。结果 观察组手术时间、术中出血量、术后肛门排气时间、下床活动时间、住院时间均小于对照组，差异有统计学意义（P＜0.05）；观察组术后VAS评分低于对照组，差异有统计学意义（P＜0.05）；观察组生活质量各维度评分均高于对照组，差异有统计学意义（P＜0.05）；观察组并发症发生率为8.11%，低于对照组的16.21%，差异有统计学意义（P＜0.05）；随访6个月，观察组复发率为5.41%，与对照组的8.11%，差异无统计学意义（P＞0.05）。结论 腹腔镜下完全腹膜外疝修补术治疗成人腹股沟疝效果理想，具有手术时间短、术中出血量少、住院时间短的优势，且可有效降低患者疼痛程度，促进术后恢复，提高生活质量，降低并发症发生率及复发率，具有良好的远期效果。
Abstract:
Objective To study the clinical effect of laparoscopic total extraperitoneal hernia repair in the treatment of adult inguinal hernia.Methods A total of 74 adult inguinal hernia patients treated in our hospital from April 2019 to April 2020 were selected as the research objects. They were divided into control group and observation group by random number table method, with 37 cases in each group.The control group was treated with open-filled tension-free hernia repair, and the observation group was treated with laparoscopic complete extraperitoneal hernia repair.The operation indexes, VAS score, complication rate and recurrence rate of the two groups were compared.Results The operation time, intraoperative blood loss, postoperative anus exhaust time, time to get out of bed, and hospital stay in the observation group were less than those in the control group, the difference was statistically significant (P<0.05);The postoperative VAS score of the observation group was lower than that of the control group,the difference was statistically significant (P＜0.05);The scores of all dimensions of the quality of life of the observation group were higher than those of the control group,the difference was statistically significant (P＜0.05);The complication rate in the observation group was 8.11%, which was lower than 16.21% in the control group,the difference was statistically significant(P＜0.05);Follow up for 6 months,the recurrence rate in the observation group was 5.41%, which was not statistically different from 8.11% in the control group(P>0.05).Conclusion Laparoscopic total extraperitoneal hernia repair is effective in the treatment of adult inguinal hernia.It has the advantages of short operation time, less intraoperative blood loss, and short hospital stay. It can effectively reduce the pain of patients, promote postoperative recovery, improve the quality of life, reduce the incidence of complications and recurrence, and has a good long-term effect.
