摘要:
目的 观察显微镜外科手术治疗脑胶质瘤的近期疗效及对患者认知功能的影响。方法 选取2016年6月-2020年6月天津市宝坻区人民医院神经外科收治的71例脑胶质瘤患者,依据治疗方案的不同将采用显微镜外科手术治疗的40例设为观察组,采用常规开颅切除术治疗的31例设为对照组。比较两组围术期指标(术中出血量、切口长度、手术时间、住院时间)、蒙特利尔认知评估量表(MoCA)评分、日常生活活动能力量表(ADL)评分、近期疗效、术后并发症及复发情况。结果 观察组术中出血量及切口长度均小于对照组,手术时间、住院时间短于对照组,差异有统计学意义(P<0.05);观察组治疗有效率高于对照组,差异有统计学意义(P<0.05);术后6个月,两组MoCA、ADL评分均有升高,且观察组高于对照组,差异有统计学意义(P<0.05);观察组术后并发症及复发率均小于对照组,差异有统计学意义(P<0.05)。结论 显微镜外科手术治疗脑胶质瘤的近期疗效确切,可有效提升患者的认知功能及生活能力,且创伤小、并发症少、复发率低。
Abstract:
Objective To observe the short-term efficacy of microsurgical treatment of brain glioma and its influence on cognitive function of patients.Methods A total of 71 patients with brain glioma admitted to the Department of Neurosurgery of Tianjin Baodi District People’s Hospital from June 2016 to June 2020 were selected. According to the different treatment regimens, 40 patients treated with microscopic surgery were set as the observation group, and 31 patients treated with conventional craniotomy were set as the control group. Perioperative indicators (intraoperative blood loss, incision length, operation time, hospitalization time), Montreal Cognitive Assessment Scale (MoCA) score, Activity of Daily Living Scale (ADL) score, short-term efficacy, postoperative complications and recurrence were compared between the two groups.Results The intraoperative blood loss and incision length in the observation group were less than those in the control group, and the operation time and hospitalization time were shorter than those in the control group, the differences were statistically significant (P<0.05). The effective rate of the observation group was higher than that of the control group, and the differences were statistically significant (P<0.05). At 6 months after operation, the scores of MoCA and ADL in the two groups increased, and those in the observation group were higher than the control group, the differences were statistically significant (P<0.05). The postoperative complications and recurrence rate of the observation group were lower than those of the control group, and the differences were statistically significant (P<0.05).Conclusion Microscopic surgery has a significant short-term curative effect in the treatment of brain glioma, which can effectively improve the cognitive function and living ability of patients, with less trauma, fewer complications, and low recurrence rate.
