摘要:
目的 探讨替格瑞洛联合阿司匹林、阿托伐他汀钙片治疗不稳定性心绞痛的疗效及对主要不良心血管事件的影响。方法 选取2021年1月-2023年12月石城县人民医院收治的60例不稳定性心绞痛患者作为研究对象，按照随机数字表法将其分为对照组和研究组，每组30例。对照组应用阿司匹林、阿托伐他汀钙片联合氯吡格雷治疗，研究组应用替格瑞洛、阿司匹林、阿托伐他汀钙片治疗。比较两组临床疗效、心功能[射血分数（EF）、每搏输出量（SV）、短轴缩短率（FS）、左室舒张末期内径（LVDd）]、血小板水平（血小板计数、血小板抑制率）、炎性因子水平[超敏C反应蛋白（hs-CRP）、白介素-6（IL-6）]、主要心血管不良事件（心力衰竭、心源性死亡、心肌梗死、再发心绞痛）发生情况。结果 与对照组治疗总有效率的76.67%比较，研究组治疗总有效率更高，达96.67%（P＜0.05）；两组治疗后LVDd水平均低于治疗前，而SV、FS、EF水平均高于治疗前，且与对照组比较，研究组LVDd水平更低，而SV、FS、EF水平更高（P＜0.05）；两组治疗后血小板计数、血小板抑制率均高于治疗前，而hs-CRP、IL-6均低于治疗前，且与对照组比较，研究组治疗后血小板计数、血小板抑制率均更高，而hs-CRP、IL-6均更低（P＜0.05）；与对照组主要心血管不良事件发生率的23.33%比较，研究组主要心血管不良事件发生率更低，仅为3.33%（P＜0.05）。结论 替格瑞洛联合阿司匹林、阿托伐他汀钙片治疗不稳定性心绞痛的疗效确切，能够改善患者心功能及血小板指标，减轻患者炎性反应，降低主要不良心血管事件发生率，值得临床应用。
Abstract:
Objective To investigate the efficacy of ticagrelor combined with aspirin and atorvastatin calcium tablets in the treatment of unstable angina pectoris and its effect on major adverse cardiovascular events. Methods A total of 60 patients with unstable angina pectoris admitted to Shicheng County People’s Hospital from January 2021 to December 2023 were selected as the research objects. According to the random number table method, they were divided into control group and study group, with 30 patients in each group. The control group was treated with aspirin, atorvastatin calcium tablets combined with clopidogrel, and the study group was treated with ticagrelor, aspirin and atorvastatin calcium tablets. The clinical efficacy, cardiac function [ejection fraction (EF), stroke volume (SV), fractional shortening (FS), left ventricular end-diastolic diameter (LVDd)], platelet level (platelet count, platelet inhibition rate), inflammatory factor level [high-sensitivity C-reactive protein (hs-CRP), interleukin-6 (IL-6)], and major cardiovascular adverse events (heart failure, cardiogenic death, myocardial infarction, recurrent angina) were compared between the two groups. Results Compared with the total effective rate of 76.67% in the control group, the total effective rate of the study group was higher, reaching 96.67% (P<0.05). After treatment, the level of LVDd in the two groups was lower than that before treatment, while the levels of SV, FS and EF were higher than those before treatment, meanwhile compared with the control group, the level of LVDd in the study group was lower, while the levels of SV, FS and EF were higher (P<0.05). After treatment, the platelet count and platelet inhibition rate of the two groups were higher than those before treatment, while hs-CRP and IL-6 were lower than those before treatment, meanwhile compared with the control group, the platelet count and platelet inhibition rate of the study group were higher after treatment, while hs-CRP and IL-6 were lower (P<0.05). Compared with 23.33% of the incidence of major cardiovascular adverse events in the control group, the incidence of major cardiovascular adverse events in the study group was lower, only 3.33% (P<0.05). Conclusion Ticagrelor combined with aspirin and atorvastatin calcium tablets is effective in the treatment of unstable angina pectoris, which can improve the cardiac function and platelet index, reduce the inflammatory response, and reduce the incidence of major adverse cardiovascular events. It is worthy of clinical application.
