摘要:
目的 观察乌司他丁对体外循环二尖瓣置换术患者肺功能损伤及炎性因子的调节效果。方法 选择2017年5月~2018年8月我院收治80例行体外循环二尖瓣置换术治疗的患者作为研究对象,随机分为观察组与对照组,每组40例。对照组给予生理盐水治疗,观察组患者给予乌司他丁治疗,比较两组围术期不同时间点[麻醉诱导切皮前(T1)、体外循环结束后1 h(T2)、体外循环结束后4 h(T3)]肺泡气-动脉血氧分压差(A-aDO2)与炎性因子水平[肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)水平]。结果 两组T1时A-aDO2、TNF-α、IL-6、IL-10水平比较,差异无统计学意义(P>0.05);观察组T2、T3时A-aDO2值、TNF-α、IL-6水平均低于对照组,IL-10水平高于对照组,差异有统计学意义(P<0.05)。结论 乌司他丁治疗体外循环二尖瓣置换术患者可在一定程度上减轻肺功能损伤,降低炎性因子水平。
Abstract:
Objective To observe the effect of ulinastatin on lung function injury and inflammatory factors in patients undergoing mitral valve replacement with cardiopulmonary bypass.Methods From May 2017 to August 2018, 80 patients treated with mitral valve replacement by extracorporeal circulation in our hospital were selected as the research object, and using the random method to divided into observation group and control group, with 40 cases in each group. The control group was given normal saline, and the observation group was given ulinastatin, and the two groups were compared at different time points during the perioperative period [before anesthesia induced skin incision (T1), 1 h after the end of extracorporeal circulation (T2), after the end of extracorporeal circulation 4 h (T3)] Alveolar gas-arterial blood oxygen partial pressure difference (A-aDO2) and inflammatory factor levels [tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin prime-10 (IL-10) level].Results The levels of A-aDO2, TNF-α, IL-6 and IL-10 at T1 were compared between the two groups, the difference was not statistically significant (P>0.05); The levels of A-aDO2, TNF-α and IL-6 in the observation group at T2 and T3 were lower than those in the control group, and the level of IL-10 was higher than that in the control group, the difference was statistically significant (P<0.05).Conclusion Ulinastatin can reduce lung injury and reduce the level of inflammatory factors to a certain extent in patients undergoing mitral valve replacement with cardiopulmonary bypass.
