摘要:
目的 观察托拉塞米联合多巴胺持续静脉泵入治疗利尿剂抵抗的老年心力衰竭的效果。方法 选取2018年5月~2019年5月在我院诊治的86例利尿剂抵抗性老年心力衰竭患者为研究对象，采用随机数字表法分为对照组和观察组，各43例。对照组采用呋塞米联合多巴胺持续静脉泵入治疗，观察组采用托拉塞米联合多巴胺持续静脉泵入治疗，比较两组临床治疗总有效率、治疗前后心功能指标（心率、射血分数）、24h尿量、血脑钠肽。结果 观察组治疗总有效率为74.41%，高于对照组的58.13%，差异有统计学意义（P＜0.05）；治疗后两组心率低于治疗前，且观察组低于对照组，差异有统计学意义（P＜0.05）；治疗后两组射血分数、24h尿量均大于治疗前，且观察组大于对照组，差异有统计学意义（P＜0.05）；治疗后两组血脑钠肽水平均低于治疗前，且观察组低于对照组，差异有统计学意义（P＜0.05）。结论 托拉塞米联合多巴胺持续静脉泵入治疗利尿剂抵抗性老年心力衰竭疗效确切，有助于降低心率，提高射血分数，增加24h尿量，且降低血脑钠肽水平。
Abstract:
Objective To observe the effect of torsemide combined with dopamine continuous intravenous pumping on diuretic resistant elderly heart failure.Methods A total of 86 diuretic-resistant elderly heart failure patients diagnosed and treated in our hospital from May 2018 to May 2019 were selected as the research object. They were divided into a control group and an observation group with a random number table method, 43 cases each. The control group was treated with furosemide combined with dopamine continuous intravenous pumping, and the observation group was treated with torasemide combined with dopamine continuous intravenous pumping to compare the total clinical effectiveness of the two groups, the cardiac function indexes (heart rate, ejection fraction), 24h urine output, blood brain natriuretic peptide.Results The total effective rate of treatment in the observation group was 74.41%, higher than 58.13% in the control group, the difference was statistically significant (P<0.05); after treatment, the heart rate of the two groups was lower than before treatment, and the observation group was lower than the control group,the difference was statistically significant(P<0.05); After treatment, the ejection fraction and 24h urine volume of both groups were greater than before treatment, and the observation group was greater than the control group, the difference was statistically significant (P<0.05); the blood natriuretic peptide level of the two groups after treatment was lower than before treatment, and the observation group was lower than the control group,the difference was statistically significant(P<0.05). Conclusion Torasemide combined with dopamine continuous intravenous pumping is effective in the treatment of diuretic-resistant elderly heart failure, which can help to reduce heart rate, increase ejection fraction, increase 24h urine output, and reduce blood natriuretic peptide levels.
