摘要:
目的 探讨胃癌手术后继发急性小肠扭转的病因及诊治原则。方法 回顾性分析2011年4月~2020年4月我院收治的7例胃癌根治术后继发急性小肠扭转患者的临床资料，分析其手术方法及临床结局。结果 5例为全胃切除术，2例为远端胃部分切除术（Billroth-Ⅱ），入院诊断均考虑小肠扭转，均行手术治疗并证实为肠扭转，其中6例患者康复出院，术后随访至今均无复发，1例患者术后24 h因感染性休克死亡。结论 胃癌术后继发急性小肠扭转病情发展迅速，早期诊断和及时手术是治疗的关键。
Abstract:
Objective To investigate the etiology and principles of diagnosis and treatment of acute small bowel volvulus after gastric cancer surgery. Methods A retrospective analysis of the clinical data of 7 patients with acute small bowel volvulus after radical resection of gastric cancer admitted to our hospital from April 2011 to April 2020, and analyzed their surgical methods and clinical outcomes.Results 5 cases underwent total gastrectomy and 2 under partial distal gastrectomy (Billroth-Ⅱ). Small intestinal volvulus was considered in the diagnosis of admission. All patients were treated with surgical treatment and proved to be intestinal volvulus. 6 patients recovered and were discharged after surgery. There had been no recurrence since follow-up, and 1 patient died of septic shock 24 h after operation.Conclusion The acute small bowel volvulus secondary to gastric cancer develops rapidly. Early diagnosis and timely surgery were the key to treatment.
