摘要:
目的 比较普通钢板与锁定钢板内固定治疗复杂胫骨平台骨折的效果。方法 选取2014年5月~2019年5月我院收治为复杂胫骨平台骨折患者80例作为研究对象，根据手术内固定物不同分为对照组（36例）和观察组（44例）。对照组采取普通钢板内固定治疗，观察组采取锁定钢板内固定治疗，比较两组手术时间、住院时间、手术出血量、手术并发症、术后患肢开始负重训练时间、骨折愈合时间以及患肢膝关节术后功能康复效果。结果 观察组手术时间、住院时间均长于对照组，术中出血量低于对照组，但差异无统计学意义（P＞0.05）；观察组术后开始负重训练时间、骨折愈合时间短于对照组，差异有统计学意义（P＜0.05）；观察组并发症发生率为15.91%、膝关节功能康复优良率为95.45%，优于对照组的33.33%、75.00%，差异有统计学意义（P＜0.05）。结论 锁定钢板内固定治疗复杂胫骨平台骨折患者可使患者尽早开始负重训练，缩短骨折愈合时间，促进患肢膝关节功能恢复，降低手术并发症，治疗效果优于普通钢板内固定治疗。
Abstract:
Objective To compare the effects of common plate and locking plate internal fixation in the treatment of complex tibial plateau fractures. Methods A total of 80 patients with complex tibial plateau fractures admitted to our hospital from May 2014 to May 2019 were selected as the research objects, and were divided into control group (36 cases) and observation group (44 cases) according to different surgical internal fixators. The control group was treated with ordinary plate internal fixation, and the observation group was treated with locking plate internal fixation. The operation time, hospitalization time, surgical blood loss, surgical complications, postoperative weight-bearing training time of the affected limb, fracture healing time and affected limb were compared between the two groups Functional rehabilitation effect after knee surgery.Results The operation time and hospital stay of the observation group were longer than those of the control group, and the amount of intraoperative blood loss was lower than that of the control group, but the difference was not statistically significant (P>0.05); the observation group started weight training time and fracture healing time was shorter than the control group,the difference was statistically significant (P<0.05); the complication rate of the observation group was 15.91%, and the excellent and good rate of knee function rehabilitation was 95.45%, which was better than 33.33% and 75.00% of the control group,the difference was statistically significant (P<0.05).Conclusion Locking plate internal fixation for the treatment of complex tibial plateau fractures could enable patients to start weight-bearing training as soon as possible, shorten fracture healing time, promote the recovery of knee joint function of the affected limb, and reduce surgical complications. The treatment effect was better than ordinary plate internal fixation.
