摘要:
目的 探讨胫骨近端外侧L型钢板与高尔夫球棒形钢板内固定治疗胫骨外侧平台骨折的血清炎性指标水平及对骨折预后的影响。方法 回顾性分析2015年1月~2020年9月我院收治的胫骨外侧平台骨折患者40例,根据手术类型不同分为观察组和对照组,观察组19例,对照组21例,观察组接受胫骨近端外侧L型钢板固定治疗,对照组接受高尔夫球棒形钢板固定治疗,比较两组血清学指标、住院时间、并发症发生情况、临床及功能评分、Rasmunssen评分。结果 术前、术后7天两组CRP、ESR、PCT比较,差异均无统计学意义(P>0.05),术后1、3天,观察组CRP、ESR、PCT均低于对照组,差异有统计学意义(P<0.05);观察组住院时间低于对照组,差异有统计学意义(P<0.05),对照组并发症发生率为9.52%,与观察组的15.79%比较,差异无统计学意义(P>0.05);观察组疼痛、缺陷评分低于对照组,稳定性、活动范围、行走情况、上下楼梯情况、功能缺陷评分均高于对照组,差异有统计学意义(P<0.05);两组术后Rasmunssen评分比较,差异无统计学意义(P>0.05)。结论 胫骨近端外侧L型钢板固定治疗胫骨外侧平台骨折患者疗效较好,有助于改善血清学指标,缩短住院时间,改善患者关节功能,且不增加并发症发生情况。
Abstract:
Objective To investigate the level of serum inflammatory indexes in the treatment of lateral tibial plateau fractures with L-shaped plates and golf club-shaped plates on the proximal side of the tibia and their effects on the prognosis of fractures.Methods A retrospective analysis of 40 patients with lateral tibial plateau fractures admitted to our hospital from January 2015 to September 2020.According to different types of surgery, they were divided into observation group and control group. There were 19 cases in the observation group and 21 cases in the control group. The observation group received L-shaped plate fixation treatment on the proximal side of the tibia, and the control group received golf club-shaped plate fixation treatment.Serological indicators, hospital stay, complications, clinical and functional scores, and Rasmunssen scores were compared between the two groups.Results There was no significant difference in CRP, ESR, and PCT between the two groups before and 7 d after operation (P>0.05).The CRP, ESR, and PCT of the observation group were lower than those of the control group on the 1st and 3rd day after operation, the difference was statistically significant (P<0.05);The length of stay in the observation group was lower than that of the control group,the difference was statistically significant (P<0.05).The complication rate in the control group was 9.52%, which was not statistically significant compared with 15.79% in the observation group (P>0.05);The pain and defect scores of the observation group were lower than those of the control group, and the scores of stability, range of motion, walking, stairs, and functional defects were all higher than those of the control group,the difference was statistically significant (P<0.05);There was no statistically significant difference in the Rasmunssen scores between the two groups after surgery (P>0.05).Conclusion L-shaped plate fixation of proximal lateral tibial plateau fractures has a good curative effect, which can help to improve the serological indicators, shorten the length of hospital stay, improve the joint function of patients, and does not increase the incidence of complications.
