目的 探讨脂联素浓度、脂联素基因第2外显子的+45T/G(rs2241766)和第2内含子的+276G/T(rs1501299)两个多态性位点、PCOS伴胰岛素抵抗(IR)三者的关系,并探讨其是否可以为临床诊疗及预后提供数据依据。方法 收集2017年1月~2019年1月昆山市中医医院就诊年龄在20~35岁,符合鹿特丹标准的PCOS患者和正常人各100例。采用分子生物学的方法对脂联素基因进行分型。比较两组临床资料、脂联素浓度及脂联素基因分型。结果 PCOS组的雄激素、雌激素、促黄体生成素、胰岛素、胆固醇、甘油三酯、血糖高于对照组(P<0.05);对照组的促卵泡生成素、泌乳素低于 PCOS 组(P<0.05);PCOS组脂联素水平和IR存在线性关系(P<0.05);对照组脂联素基因多态位点+276G/T( rs1501299)不符合 HWE 遗传定律,组间无法比较;两组+45T/G(rs2241766)位点的基因型(TT、TG、GG)频率分布比较,差异无统计学意义(P>0.05),此位点基因型频率与脂联素浓度、HOMA-IR进行关联分析,差异无统计学意义(P>0.05)。结论 PCOS 组的脂联素水平和IR呈负相关,脂联素基因+45T/G(rs2241766)位点基因分型与PCOS伴IR患者无显著关系,可能不参与 PCOS 发生。
Abstract:
Objective To investigate the adiponectin concentration, the +45T/G (rs2241766) and +276G/T (rs1501299) polymorphisms in the second exon of the adiponectin gene, and PCOS with insulin resistance (IR) The relationship between the three and explore whether it can provide data for clinical diagnosis and treatment and prognosis.Methods From January 2017 to January 2019, 100 PCOS patients and 100 normal people who were 20-35 years old and met the Rotterdam standard were collected from Kunshan Traditional Chinese Medicine Hospital. Adiponectin genes were typed using molecular biology methods. The clinical data, adiponectin concentration and genotype of adiponectin were compared between the two groups. Results Androgen, estrogen, luteinizing hormone, insulin, cholesterol, triglyceride, and blood sugar in the PCOS group were higher than those in the control group, the difference was statistically significant (P<0.05); the control group had low follicle stimulating hormone and prolactin the PCOS group, the difference was statistically significant (P<0.05); there was a linear relationship between adiponectin levels and IR in the PCOS group (P<0.05); Adiponectin gene polymorphic site +276G/T (rs1501299) in the control group does not comply with the HWE genetic law and cannot be compared between groups; the frequency of genotypes (TT, TG, GG) at +45T/G (rs2241766) sites in the two groups the distribution comparison showed no statistically significant difference (P>0.05). The correlation analysis between the genotype frequency of this locus and the adiponectin concentration and HOMA-IR showed that the difference was not statistically significant (P>0.05).Conclusion The level of adiponectin in the PCOS group is negatively correlated with IR. The genotyping of adiponectin gene +45T/G (rs2241766) has no significant relationship with patients with PCOS with IR, and may not be involved in the occurrence of PCOS.
